Description:
Social services case management position in the PACE Social Work Department assisting participants with various "social determinants of health" (SDOH) needs including housing, food insecurity, benefits and more. Work closely with a supportive interdisciplinary medical team supporting the PACE participants and their families.
Join PACE! As part of a dynamic Interdisciplinary Team (IDT) in an innovative model of care, you will work closely with participants and their caregivers to help them continue to live safely in their homes and communities as independently as possible. You will work in an integrated primary care and behavioral health setting to support participants in their homes and at one of our PACE centers. Full-time position to act as case manager and support for the Neighborhood PACE Social Work department. Work with older adult participants and families to assist with vital "SDOH" (Social Determinants of Health) issues including housing, food insecurity, benefits access, and more. An ideal candidate will be knowledgeable about community resources, be detail oriented, able to multitask, and possess excellent communication and computer skills. Must hold a Bachelor’s degree in social work or related field OR have two years of relevant work experience in a community setting. Bilingual (English/Spanish) required. Automobile required.
Participant Navigator Responsibilities
Builds trusting relationships with participants
Provides assistance with housing applications; maintains records of pending applications; provides follow-up on applications
Acts as liaison and provides housing advocacy with various housing organizations on behalf of participants
Assists with case management tasks including, but not limited to: Food Stamps/DTA applications and re-certifications, Safelink/Assurance phone assistance, scheduling RIDE interviews, Social Security income verifications, calling Social Security to troubleshoot issues
Researches and provides participants with resource information, such as food pantries, support groups, immigration assistance
Provides money management assistance and act as liaison to representative-payees
Implements supportive interventions such as check –in calls under the direction of the social worker
Performs community outreach and accompanies participants with exceptional hardship and need to appointments in special circumstances.
Consults appropriately with clinical staff
Thoroughly documents all communication with participants in the medical record in a timely manner
Demonstrates excellent customer services skills and cultural competency with participants and caregivers of diverse backgrounds
Demonstrates strong commitment to participants, resolves case management issues as expeditiously as possible to participant satisfaction. As representative of PACE and EBNHC, maintains a highly professional, courteous and respectful manner with participants, caregivers and colleagues
