State of Massachusetts Transition Service Coordinator in New Bedford, Massachusetts

The Department of Developmental Services (DDS) Southeast Region is seeking a person with strong attributes in the Human Service field to work in theNew Bedford Area Office in New Bedford, MAas aHuman Service Coordinator (A/B) (HSC). This position will specifically work with those individuals who are transitioning from the schools to adult services. This specialized case load requires that the selected candidate maintains ongoing and frequent communication with families, school systems and other children services programs. They should be knowledgeable of the changes that take place when leaving the school system and entering adult services. This candidate should be able to help guide family members through all the processes that they need to complete in order to have a successful transition. This entails having a good working knowledge of systems such as SSI, Guardianship and Housing as well as current adult programs that would be available and appropriate for the individual transitioning. Regular contact with individuals, families, and providers is a necessity for cohesiveness in working with the team. Along with transition and long term planning help, the selected candidate will be required to arrange for and provide crisis intervention, when needed, as well as coordination of team meetings to ensure clear communication and follow up planning. The selected candidate will be required to travel to individuals’ homes, programs, and schools and outside meetings that may or may not be in close proximity of the Area Office. It is essential for this position to be responsible for the coordination, facilitation, development and progress of transition plans for assigned individuals as per the Department of Developmental Services (DDS) regulations. They are also responsible for the coordination of DDS funded and at times some auxiliary support services in response to individual’s needs during this transition time. The HSC will also be required to coordinate referrals to community services and other state agencies, as well as provide outreach and communication with these entities. The selected candidate must demonstrate a willingness to work with individuals and families on their caseloads who are interested in participating in the Participant Directed Program and/or Agency With Choice. This will involve learning the nuts and bolts of Participant Directed Program and/or Agency With Choice in order to help families and individuals make informed decision about directing their own supports and assisting them in this process. The HSC must maintain up to-date record keeping - both electronic and hard copy, maintain appropriate and timely documentation for individuals on their caseload, communicate with team members, as well as maintain regular communication with Area Office Management. It is essential to effectively represent the Department and Area Office in a professional manner. Other job duties may be assigned as needed.
