Reports to the Hospice Clinical Manager with responsibility to facilitate and promote the acceptance of appropriate patients to Hospice and Palliative Care and other Baystate Home Health Care entities.
Assesses and evaluates patients for home health care needs and coordinates referrals for services. Builds and maintains positive relationships with assigned referral sources. Participates in marketing activities to make BVNAH a preferred provider for home health services.
Provide on-site evaluation, meet with patient, and complete chart review of all referred patients, determining appropriateness and level of priority for admission to BVNAH services. Partners with intake and clinical on capacity management to promote a smooth transition to BVNAH policies. Obtain and prepare referral information that includes verified demographic information, physician verification, identified vendor when applicable, confirmed physician orders, discharge medications, and payor information.
Build and maintain positive relationships with referral sources
Meet with assigned referral sources on a scheduled basis and document meeting content. Responds promptly to referral source issues, documenting customer service actions, and notify Hospice Clinical Operations Manager or designee.
Participate in marketing activities to potential referral sources within and outside Baystate Health facilities
Registered and Licensed Nurse in the State of Massachusetts.
3 years nursing experience
Must have knowledge of hospice and palliative care principles of case management and strong background in end of life care.
Excellent judgment, communication and computer skills.
Ability to work independently with attention to detail.
Knowledge of home care regulations and standards, payor specific information
Knowledge of and proper use of Home care software for referral processes while following intake guidelines
Demonstrated ability to be a team player and work within an interdisciplinary team.
Valid Driver’s License - Must have a car and a valid Massachusetts driver’s license. Motor vehicle report will be reviewed and hiring decision will be based on information obtained

Location -- 50 Maple Street Baystate Visiting Nurse Association & Hospice
40 Hours per week
Monday - Friday 8:30-5 or 9-5:30
This position will provide coverage to facilities and MD offices and will be cross trained to cover the hospital
Possible rotation to weekends in the future.
