Community Healthlink RN-Fitchburg Leominster or Worcester Program for Assertive Community Treatment in Worcester, Massachusetts
The Program for Assertive Community Treatment and PACT FLoW (Fitchburg, Leominster or Worcester).
This program assists seriously mentally ill adults who need intensive treatment by reducing hospitalizations and providing vocational and community support.
This is a Department of Mental Health (DMH) closed referral program.
Candidate is responsible for coordinating medications for the clients in the program and working with the psychiatrist and pharmacy to coordinate medications
Candidate will be responsible for assisting clients will setting up and going to medical appointments along with collaborating with collateral medical providers surrounding client's needs
Responsible for a client case load, are responsible for treatment plans daily notes and corresponding with outside agencies.
Collaborate with outside agencies regarding medical concerns.
Assist with ADL support such as grocery shopping running errands and connecting clients to community resources.
The RN collaborates with the pharmacy and coordinates with the entire team surrounding client’s medication needs
Associates or bachelor's degree in nursing
Current valid US-issued driver's license and ability to provide registered and insured automobile for work related purposes.
Psychiatric experience preferred but will train if candidate has an interest in psychiatric mental health
Must be able to pass a CORI background check
