The Bridge of Central Massachusetts, Inc. Treatment Coordinator - Oberlin Street (Worcester) in Worcester, Massachusetts
This intensive residence provides a comprehensive, therapeutic living environment for eight men and women with major mental health challenges. Intensive staff support is available 24 hours a day. The purpose of the program is to help adults manage their illness, develop independent living skills, and live successfully in the community. This program offers Dialectical Behavior Therapy (DBT) and Peer Support and utilizes Supported Employment and Illness Management and Recovery (IMR) as the treatment models.
The Treatment Coordinator is responsible for assisting the Clinician and Program Manager with developing and writing treatment and behavior plans, and will help facilitate groups. The Treatment Coordinator will be expected to track the timely completion of documentation. The Treatment Coordinator will be responsible for maintaining collateral contacts, assisting clinicians with assessments, and assisting with the scheduling and facilitation of Person Centered Planning meetings. The Treatment Coordinator also provides direct care to individuals, shift supervision, group facilitation, serves as a member of the leadership team, and has division on-call responsibilities.
Bachelor's Degree in Social Services related field preferred, plus a minimum of one to two years of experience working with adults living with serious and persistent mental illness. Candidate must have knowledge of writing treatment plans and assessments. Candidate must be at least 21 years of age, have a valid driver's license, and be able to demonstrate proficiency with Microsoft Word. Candidate must be First Aid, CPR, and Medication Administration Program (MAP) certified or certifiable (The Bridge provides MAP Training).
