Craig's Doors Outreach Case Manager in Amherst, Massachusetts

The Outreach Case Manager at Craig\'s Doors plays a crucial role in providing comprehensive case management services to adults 18 and older experiencing a housing crisis. Working primarily from the Craig\'s Doors Resource Center in Amherst, MA, the Outreach Case Manager conducts assessments, develops service plans, connects clients to resources, and provides ongoing support as they work towards stabilization. This position requires strong interpersonal skills, knowledge of community resources, and a commitment to trauma-informed care. Responsibilities: Outreach Services: ? Conduct outreach activities to engage individuals experiencing homelessness in the community. ? Build rapport and trust with clients, demonstrating empathy, respect, and understanding. ? Provide information about available services and resources offered by Craig\'s Doors. ? Provide orientation to Day Program services for community members attending the Craig\'s Doors Resource Center. Client-Centered Support ? Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client centered. ? Conduct comprehensive evaluation and assessment of client needs, strengths, limitations, barriers, and goals. ? Utilize motivational interviewing techniques to empower guests and promote positive change. ? Work with individuals to develop realistic service plans that identify issues and map a course towards their resolution. Provide long-term support as they work towards stabilization. Service plans may include assistance with housing, employment, income, education, health care, mental health, substance use disorders, disability support, domestic violence, legal and credit issues, or a combination of the above. ? Provide consistent, reliable support and guidance to clients as they work towards achieving their goals. ? Provide transportation to appointments using Craig\'s Doors van, as necessary. Housing Support: ? Assist clients with housing search, including completing applications for public housing, vouchers, private housing, and supportive housing. ? Provide coaching and skill-building to enhance housing readiness, including financial management and training programs. Resource Connection ? Connect clients to community resources such as food pantries, medical facilities, governmental offices, and public transportation. ? Assist with applications for services such as Veteran?s benefits, MassHealth, Social Security, SNAP, and emergency assistance programs. Relationship Building and Conflict Resolution ? Build relationships with clients? landlords, acting as a liaison and advocate. Be a first point of contact in resolving landlord or neighbor conflicts amicably. Create response plans in event of health crises. ? Provide or arrange for training on tenant?s rights, financial success, and other activities that support long-term sustainability. ? Initiate new community partnerships and support clients in establishing new social relationships. ? Advocate for clients rights and needs with landlords, emergency responders, and other agencies. ? Provide crisis intervention to support clients, bringing in outside agencies when necessary. Documentation and Reporting ? Maintain accurate, comprehensive, and timely logs, case notes, and client records. ? Document crisis incidents for Craig?s Doors records and state reporting. ? Collect, enter, and maintain all necessary statistical data required by Craig?s Doors and our funders. Professionalism ? Continually assess, refine, and develop service plans to improve clients\' lives, collaborating with Site Managers and organization leadership. ? Adhere to professional standards, protocols, rules, and regulations governing case management services. ? Apply trauma-informed care and case management principles in all interactions with clients.
