CLINICAL CARE MANAGER

What You Will do:

The Clinical Care Manager serves as the manager of a Behavioral Health Community Partners (BH CP) team, providing direct clinical care management services to enrolled clients, hiring and supervising Care Coordinators and nursing staff and overseeing the operations of all care management and care coordination activities within the team. The position ensures that team members are properly trained and are providing person-centered support to Enrollees in compliance with all contractual and Edinburg Center requirements. The position provides both clinical and operational supervision to all team members and serves as the designated point of contact for referrals to the team.

Who You Are:

Master’s Degree with a Massachusetts license in a related discipline preferred with at least 2 years of experience working with people with serious mental illness and substance use disorders required. Prior supervisory experience required. Must be familiar with and embraces person-centered planning philosophy and motivational interviewing. Ability to use a computer for word processing, EHR and email/Outlook Calendar, and the Internet for resource searches and assisting clients with online surveys and using online decision-making tools required. Regular access to reliable transportation required.
