What You'll Do:
The Certified Diabetes Care and Education Specialist (CDCES) will work directly with patients to provide diabetes education and support with the goal of improving adherence and health outcomes. This remote position is responsible for consistently meeting program metrics while also meeting quality requirements. The CDCES position is a part-time and independent contractor role.
Primary Responsibilities:
Provide diabetes education support to patients with the goal of improving adherence and health outcomes
Quickly establish rapport and therapeutic alliances via phone with a diverse population of patients
Produce behavior change among patients using motivational interviewing techniques.
Present online webinars
Consistently meet call metrics while also meeting quality requirements
Other duties as assigned
Requirements and Experience:
Must have a current certification as a Certified Diabetes Care and Education Specialist from the National Certification Board for Diabetes Educators (NCBDE)
Must be fluent in Spanish.
Proven experience utilizing motivational interviewing to provide positive behavior change
Demonstrated ability to receive constructive feedback and then incorporate it into patient calls to drive performance
Must have a home office for private patient conversation along with high-speed internet
Able to work independently and manage time efficiently
Must be comfortable utilizing advanced technology (Electronic Medical Records, Microsoft Office, scripting applications, device data, etc.)
Demonstrated ability to recognize patient needs and adapt communication techniques to best serve a diverse patient population
Must have a flexible schedule and be able to commit to a minimum of 20 hours per week to accommodate patients
Cannot hold full-time employment elsewhere.
