Since 1985, BHCHP’s mission has been to ensure unconditionally equitable and dignified access to the highest quality health care for all individuals and families experiencing homelessness in greater Boston. Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. We are committed to ensuring that every one of these individuals has access to comprehensive health care, from preventative dental care to cancer treatment. Our clinicians, case managers, and behavioral health professionals work in more than 30 locations to serve some of our community’s most vulnerable—and most resilient—citizens.

BHCHP is launching a brand new, full-time interdisciplinary clinic on the grounds of Massachusetts General Hospital. This clinic, the only such clinic in the USA located at an academic medical center, will aim to provide comprehensive medical, substance use and mental health services to people who are or have been homeless and wish to receive care at MGH. Further, the clinic will catalyze the development of an inpatient consult team that will see inpatients and emergency room patients who are experiencing homelessness at the bedside, helping to improve the inpatient experience of patients, and assisting teams in developing appropriate care plans.

Job Summary:  

The MGH Clinic team is looking for a hard-working and solutions-oriented individual who is excited to provide medical, housing, and general social support to primary care patients at BHCHP. This case management role is a unique fusion of clinic-based support for the MGH clinic patients as well as housing focused case management support for hospital in-patients who are experiencing homelessness as part of the inpatient homeless consult team.

We are seeking an individual who is eager to support the multi-disciplinary team based out of the MGH Clinic and is comfortable with flexibility in the workplace.

Hours: are M-F 8am-4:30pm, with the opportunity for one remote day. Clinic days will be Tuesday mornings and Fridays all day.

Responsibilities: 

  • Provide walk-in based case management support for non-street team patients in the MGH clinic, working alongside a multidisciplinary team of providers, nurses, and behavioral health clinicians.

  • Be knowledgeable about specific case management services including but not limited to public benefits (SSI, EAEDC, SNAP), connection to legal support, connection to behavioral health, transportation, and other local resources as relevant.

  • Carry a panel of approximately 25 patients to provide longitudinal case management support. These will be non-street team patients who choose to get their primary care in the MGH clinic.

  • Act as a member of the MGH inpatient homeless consult team to provide support to inpatients and patients in the ED who are experiencing homelessness. Specifically, your role will involve facilitating housing navigation for these patients. This can include but is not limited to:

  • Helping patients and inpatient teams ensure applications and paperwork, including vital documents, related to the housing process are up to date.

  • Joining housing specific meetings in the City of Boston

  • Identify and develop cooperative working relationships with homeless service providers in Boston for people experiencing homelessness.

  • Participate in city-wide networks of case management for homeless individuals.

  • To be successful in this role an individual must be able to:

  • Understand and navigate complex housing pathways within the Boston Area

  • Develop a strategy to effectively manage housing work embedded within a inpatient hospital setting

  • Understand the needs of populations that have experienced vulnerability, trauma, and economic issues.

  • Develop strong relationships with homeless service providers, policy makers, housing subsidy administrators and stabilization service providers.

  • Possess strong interpersonal and communication skills and confidence.

  • Able to exercise good judgment and is creative and proactive.

  • Work within a multidisciplinary team.

  • Be flexible and able to multi-task.

    Qualifications:

  • Bilingual in Spanish and English is preferred. 

  • Preferred: Knowledge of and experience with the network of services available to homeless persons; Strong understanding of the unique needs of LEP and Immigrant patients.

  • Prior case management experience a plus.

  • Comfortable working with multidisciplinary team, maintaining a good rapport with nursing/medical staff, legal organizations, and other disciplines.

  • Strong problem solving and communication skills (written and oral).

  • Refined ability to communicate professionally with employees and patients, both on the phone and in-person.

  • Organized, detail-oriented, and able to complete tasks in a timely manner.

  • Self-directed with the ability to work independently in multiple settings.

  • Flexible and adaptable to different health care delivery models.

  • Computer skills: proficient with Microsoft Office, entering narrative and other data into electronic medical records and other internet-based products.

    Compensation and Benefits:

    NOW OFFERING A $4,000 SIGN-ON BONUS

  • The compensation starts at $22.07 per hour and increases based on years of experience.

  • BHCHP full time employees are eligible for our competitive time off policy of 4 weeks’ vacation, health, dental and vision insurance, 403B retirement savings plan and employer retirement contribution, and pre-tax MBTA pass program with 40% discount. In addition, eligible employees will receive yearly increases, additional compensation of seven thousand five hundred dollars added to your base hiring rate for demonstrated bilingual proficiency and the opportunity to work with local hospitals and community health centers.
