Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. Job Summary:

NOW OFFERING A $4,000 SIGN ON BONUS

Boston Health Care for the Homeless Program is looking for a professional nurse with both administrative and clinical skills to perform the daily operations of the Southampton Shelter/Transitions clinic.  Responsibilities for this position include planning, implementing, supervising, and evaluating the delivery of patient care within established regulatory guidelines and professional nursing practice standards.  This is done through collaborative relationships with the clinic director, medical director, and clinic staff.

Hours: Full-time; 40 hours per week, Monday-Friday, Day/Evening shifts

Responsibilities:

  • Clinical Responsibilities

  • Oversee and monitor clinic operations including direct patient care; monitors and facilitates patient flow through clinic.

  • Provide triage and nursing assessments.

  • In conjunction with the clinic site director, develops and sustains an environment that supports excellence in clinical practice through planning and implementing programming that contributes to positive patient outcomes; evaluates the adherence to and effectiveness of interdisciplinary clinical standards and programs through the establishment of quality improvement activities.

  • Participates as a member of the patients’ care team; provides education to patients; develops and implements care plans, and provides care management services for high risk/ enhanced care patients, and participates in identification of, and care planning for, high risk/ enhanced care patients.

  • Attends clinic staff meetings.

  • Participates in ongoing clinical quality improvement endeavors.

  • Promotes patient self-management skills.

  • Develops patient action plan in partnership with patient.

  • Contacts patients within 24 hours of ED visit notification and within 48 hours of inpatient discharge.

  • Acts as team leader to facilitate huddles and communication among PCP and team members.

  • Coordinate care with other care providers across continuum of care.

  • Administrative Responsibilities

  • In conjunction with the clinic site director, interviews and supervises Transitions clinic nursing staff, according to established personnel policies and procedures of BHCHP.

  • In conjunction with the clinic director and medical director, develops PSI clinic policies and procedures.

  • Co-chairs clinic nursing staff meetings.

  • Coordinates with 112 clinic manager clinic staff schedules.

  • Assures completion of and tracks POC testing competencies and credentialing through SHS 112/ transitions Site Director.

  • Co-chairs and facilitates peer review meetings for clinic nurses.

  • Assures adherence to standards of care and documentation requirements by nursing staff.

  • Tracks completion of nursing collaborations.

  • Attends and participates in clinic management team meetings.

  • In collaboration with the clinic site director and develops clinic nursing orientation protocols.

  • Serves as a resource/mentor to staff regarding professional development, encouraging continuing education, and individual learning opportunities as appropriate.

  • Collaborates with nursing clinical educator to assure appropriate and timely training opportunities for clinic nursing staff.

  • Provides coverage for clinic site director in their absence.  

    Qualifications:

  • License to practice professional nursing in Massachusetts required.

  • Graduate of an accredited baccalaureate nursing program preferred.

  • Minimum of five (5) years of progressively responsible nursing experience; leadership experience with demonstrated management ability preferred.

    Compensation and Benefits:

    NOW OFFERING A $4,000 SIGN ON BONUS

  • The compensation starts at $42.60 per hour and increases based on years of experience.

  • BHCHP full time employees are eligible for our competitive time off policy of 4 weeks’ vacation, health, dental and vision insurance, 403B retirement savings plan and employer retirement contribution, and pre-tax MBTA pass program with 40% discount. In addition, eligible employees will receive yearly increases, additional compensation of seven thousand five hundred dollars added to your base hiring rate for demonstrated bilingual proficiency and the opportunity to work with local hospitals and community health centers.
