Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. Job Summary:

NOW OFFERING A $4,000 SIGN ON BONUS

Hours:  Full-time; 40 hours per week  

Come join Boston Health Care for the Homeless Program (BHCHP) as we build an exceptional care team that takes care of individuals experiencing homelessness at our main campus at 780 Albany Street and surrounding shelter-based clinics.  This position will provide comprehensive, high-quality primary, episodic and/or recuperative care at our main campus and surrounding shelter-based clinics for both primary care and walk-in services.

We are looking for a dynamic, patient-centered, proactive nurse-practitioner or physician assistant to join our team. The ideal candidate must be a self-starter and have the ability to lead a care team in collaboration with MD/DO colleagues and NP/PA peers.

Responsibilities:

  • Care for a complex and diverse patient population, provide primary and episodic/recuperative. Care. Perform physical examinations, order diagnostic studies, interpret results and initiate therapy in accordance with the Guidelines for Physician Assistants and Nurse Practitioners of BHCHP.

  • Refer patients as appropriate to medical specialists, hospital admission, recuperative care, and detox facilities, providing appropriate information for continuity of care.

  • Participate in collaborative relationship with BHCHP physician according to the Regulations of the Commonwealth of Massachusetts.

  • Provide education to patients on health promotion and disease treatment and prevention.

  • Work as part of multi-disciplinary team in caring for patients; may work at shelter-based clinic, hospital-based clinic, respite facility or on street-based outreach based on program needs. 

  • Maintain adequate, up-to-date medical records, completed in a timely manner, according to the guidelines of BHCHP, employing standard forms such as problem lists, medication lists, etc. ; maintain active DPH, DEA, and professional license, hospital privileges, managed care credentialing and CPR training.

  • Some on-call and holiday coverage required.

    Qualifications:

  • Licensed by the Commonwealth of Massachusetts.

  • Registered or eligible to be registered with the Massachusetts Department of Public Health and the Federal Drug Enforcement Agency.

  • At least one year of direct NP/PA experience preferred, preferably with patients who have or are experiencing homelessness, substance use disorder or severe mental illness.

  • Life support training certification (e.g. CPR, BLS, ACLS) from a course that includes a hands-on, in-person component. If not certified prior to start date, applicant must submit certification within 60 days of start date.

  • Experience in working with underserved populations, mentally ill, homeless and/ or persons with substance use disorders preferred.

  • Bilingual proficiency preferred.
