Plans, organizes, and leads work of assigned administrative population health team members in support of population health quality measures. Assigns and monitors work, working with team to develop new processes to ensure that outreach efforts are meeting stated goals as efficiently and effectively as possible.
Works with clinicians, administration, and others in order to coordinate collection of data for each population. Tracks and identifies trends with this information in order to create interventions. Coordinates the efforts of performance improvement initiatives.
Designs spreadsheets to track data and progress toward goals. Monitors and maintains accuracy and completeness of data. Designs and performs ad hoc data searches in response to internal and external requests; generates patient lists for quality improvement purposes.
Evaluates all data reports for completeness and quality of data; works with quality team to identify and correct errors.
Prepares regular data summaries and prepares reports. With assistance of quality team and project manager, troubleshoots and problem-solves unforeseen situations in data coordination that may hinder meeting goals.
Interacts with physicians and hospital support personnel to gather and retrieve patient information.
As needed, performs duties of Population Health Assistant. Performs outreach to patients, scheduling appointments, and engaging with clinical staff to ensure appropriate care is provided.
Provides administrative support, including development of educational tools and training materials for quality improvement projects and data management related to assigned population.
Provides administrative support for program development and evaluation as appropriate to meet grant and other project goals.
To qualify, you must have:
Bachelor’s degree required. Two years of additional experience may substitute for an Associate’s degree.
Internal candidates: Two years of experience as a Senior Population Health Assistant required. External candidates: Four years of comparable experience with population health data collection required.
Strong preference for work experience in a healthcare setting.
Strong verbal and written communication skills and active listening skills. Communicates clearly and appropriately.
Advanced computer navigation and keyboarding skills, including ability to multi-task and navigate between multiple systems.
Knowledge of analytical models, tools, and methods.
Ability to work with confidential information in a professional manner.
Advanced level knowledge of Microsoft Office Suite.
