Vinfen Recovery Support Navigator in Boston, Massachusetts

##Job Description The Recovery Support Navigator (RSN) provides intensive care coordination for MassHealth Members with complex medical, behavioral needs along with a history of Substance Abuse Disorder (SUD) and are enrolled in an Accountable Care Organization (ACO) or Managed Care Organization (MCO) plan. The RSN will collaborate with the Community Partner (CP) team and the clinical staff of each Enrolllee's ACO/MCO plan in order to minimize duplicative efforts in order to promote integrated care, and to ensure the quality and continuity of care. The RSN specializes in supporting Enrollees with SUD and serves as the team's resource on SUD assessment, treatment and rehabilitation techniques and resources. The role supports the Team Leader in maintaining the team's adherence to fidelity standards of co-occurring disorder (COD) evidence- based practices. This role drives outreach and engagement, assessment and care planning, care transitions, health and wellness coaching, as well as community and social services connections in partnership with Enrollees and their care teams. Job Duties and Responsibilities: The essential job duties/responsibilities of the position include but are not limited to the information listed below: * Outreach to and actively prepare Enrollees to communicate their strengths, hopes, dreams and goals, and engage in a recovery process. * Coordinate the completion of the Comprehensive Assessment (CA). * Conduct initial and ongoing risk assessment; design personal crisis management plans, relapse prevention and harm reduction strategies with members who have been identified as behaviorally complex. * Coordinate the development, implementation, and ongoing review of the Person Centered Treatment Plan (PCTP) inclusive of SUD needs and goals as well as any LTSS and / or SDH needs or goals. * Drive referrals regarding connections to any community or social services that align with the Enrollees needs and goals. * Contribute SUD expertise to team as related to SUD education, motivational enhancement, appropriate stage of change interventions, and application of evidenced-based substance addiction treatment. * Collaborate closely with PCP and other providers, including but not limited to community resources, to assure appropriate referrals based on level of care needed to optimize outcomes and minimize risk. * Implement recovery-oriented, motivational interviewing, harm-reduction, trauma-informed, culturally competent and strengths-based rehabilitation approaches * Communicate and collaborate with ACO/MCO teams and serve as a team resource. * Obtain required Prior Authorization from ACO/MCO Plan for all relevant and necessary services. * Manage all care transitions through collaboration with Enrollee, community provider staff, ICT and hospital staff to ensure a safe discharge plan and a well-coordinated implementation of that plan. * Ensure for medication review and reconciliation as triggered by a care transition or by a medication change through an outpatient medical or psychiatric visit. * Perform other duties, as required. All qualified applicants will receive consideration for employment without regard to race, color, religion, sex, pregnancy, sexual orientation, gender identity, national origin, age, protected veteran status, or disability status. ##Requirements * Bachelor's degree in Human Services related field strongly preferred, but a minimum of 5 years of experience working with co-occuring disorders, HIV/AIDS or substance addiction services may substitute * Licensure or certification as a Drug and Alcohol Counselor (LADAC II, CADAC, LADCA) in state of service strongly preferred * Extensive knowledge of available community, hospital-based, residential, day and self-help services for co-occurring disorder treatment and support. * Knowledge of health risks prevalent with adults with SMI/SUD * Knowledge of trauma-informed and culturally responsive services * Sensitivity to the cultural, religious, ethnic, disability, and gender issues Ability to make independent judgments and decisions * Ability to work in a professional and confidential capacity * Ability to work independently and as a member of a multidisciplinary team * 3-5 years Human Services experience required, with a strong preference for experience working with people with psychiatric disabilities and co-occuring disorders Driving is a requirement for this position. You must have access to a fully insured, licensed vehicle that you are willing to use in the course of your daily work.
