About the Role: Care Navigator

Care Navigator Essential Functions:

  • Works with the Care Access Manager to ensure incoming and existing clients have updated medical docs in charts

  • Serve as a brand ambassador for Vitra reflecting our vision, mission, and values

  • Show a genuine interest and compassion for the communities we serve and commitment to the diversity of our clients and team members

  • Ensures timely submission of medical documentation and authorizations requests

  • Assists with administrative tasks including answering phones, directing calls, email correspondence, photocopying, sending faxes, scanning, filing, etc.

  • Escalates client concerns and/or progress concerns to management as necessary

  • Maintains confidentiality of records in compliance with applicable laws and regulations

  • Review incoming intake paperwork, prepare all required forms, and send initial requests for medical documentation

  • Maintain database of Primary Care Provider contact information and confirm eligibility as MassHealth Providers

  • Complete a high level of calls to doctor's office and hospitals to ensure timely return of outstanding documents

  • Employ outstanding time management skills to ensure all new cases have forms sent out in a timely fashion while performing regular follow up on all outstanding requests

  • Prioritization of workload based on clients' length of time in and progress through the intake process

  • Performs any other duties as assigned

Care Navigator Requirements:

  • Bachelors degree preferred

  • Fluency in English and Spanish highly preferred

  • Previous experience in home health care or other branch of health care preferred

  • Excellent interpersonal and communication skills required

  • 2+ years prior experience with high volume record processing required

  • Creative problem-solving skills

  • Demonstrated success with data-driven outcomes

  • Customer service mentality

  • Excellent time management skills
