State of Massachusetts Adult Case Management Supervisor in Brockton, Massachusetts
The Department of Mental Health's Brockton Multi Service Center (BMSC) is seeking an experienced Adult Case Management Supervisor (Human Service Coordinator II) to join the team. The Adult Case Management Supervisor will: orient, train and provide direct supervision to case management (Human Service Coordinator I) staff; will assign individuals referred for case management to HSC I staff and oversee the provision of appropriate, quality care; monitor treatment and rehabilitation progress; provide oversight of case management services to ensure compliance with DMH Regulations, policies, and procedures; function as liaison to and provides consultation to DMH inpatient facilities, community hospitals, and service providers (DMH and non DMH); will function as contract monitor for DMH funded services as assigned. The Adult Case Management Supervisor will be responsible for promoting DMH’s philosophy of recovery, trauma informed care, person centered choices and cultural humility. _Please Note_: *This position is 40 hours per week, Work Schedule: 8:30 a.m. to 5:00 p.m. The days off are Saturday and Sunday. *_Duties and Responsibilities (these duties and responsibilities are not all inclusive):_ * Provide oversight of case management staff (HSC Is) to ensure compliance with DMH Regulations, policies, and procedures. * Conduct utilization management reviews as well as audit and approve monthly case management billing. * Review case management documentation for quality and timeliness including, but not limited, to face-to-face & collateral contacts, progress notes, comprehensive assessments, Individual Service Plans (ISPs), incident reports, screenings, referrals, discharge/transition plans, and clinical review summaries. * Assigns and reviews case management staff caseloads regularly to determine continued need for DMH services and to evaluate priority levels of individuals served based on assessed need for services. * Provides regular case supervision and consultation to each assigned case manager and maintains documentation of the supervision meeting. Assists directly with complex inter/intra agency coordination & referral. Aids staff in assessment, referral, and advocacy of clients to ensure regular and consistent support to supervised staff, makes clinical recommendations, reviews and signs off on required documentation and ensuring they meet required standards and timeframes. * Addresses any concerns that staff have related to their work and working environment and any concerns about the job performance of the reporting staff. * Ensuring compliance with Incident Reporting procedures and providing follow-up and supervision as needed. * Utilize available reports and data to monitor Case management activities and improve performance outcomes as directed. * Responds to service providers in-patient units, crisis, and community members, as the identified contact person in supervisory role. * Authorize use of benefit time and maintain adequate coverage. * Facilitate and participate in the development of plans to address risk & safety for assigned individuals according to DMH protocol and policy and in collaboration with staff, client, Supervisor, crisis intervention services & service providers on identifying historic & current risk factors to facilitate appropriate assessment and planning. * May complete screenings and assessments (e.g. suicide, substance use, etc.) as needed and according to DMH protocols and policies. * Confirms that case management staff completes appropriate screening (e.g., suicide, substance use, etc.) as needed and according to DMH policies and protocols; reviews screenings to assist with identifying individuals at risk; ensures that appropriate consultation or services are obtained, and monitors follow up. * Oversees agency onboarding and orientation of case management staff. * Conducts and completes Employee Performance Reviews (EPRS) within prescribed timelines and established guidelines. * Monitoring of assigned contracts by entering enrollments and discharges, reviewing vouchers for accuracy, reviewing and approving EIM submissions by providers, meeting with providers, identifying performance problems and offering assistance to providers in order to assure effective and efficient service delivery. * Participates in Level of Care assessments on all applicable ACCS clients within prescribed timeframes and assures that the assessments and recommendations are documented in electronic health record. * Utilize available reports and data to monitor assigned Contracts compliance, identify gaps, trends, and highlight improvement opportunities as directed. * Ensure staff respond to and manage crises by providing face-to-face or telehealth contact when appropriate. Ensures staff have direct communication with Mobile Crisis Intervention (MCI), Behavioral Health Helpline &/or 911 as well as complete necessary alerts with MCI. * Identifies information of critical concern and communicates the information to the Site Director, Clinical Director and other designees in a timely manner. * Assists in the development of transition and discharge plans with providers of inpatient level of care, assesses and tracks Section 3 requests with the Area Medical Director, Site’s Clinical Leadership, and Risk Management Team and assists in the development of diversion plans for Section 3 requests through the identification and implementation of Site and community resources whenever possible; documents information gathered as required. * Demonstrate professional leadership in internal and external meetings and facilitate clear communication and follow up. * Assists in screening and hiring of new staff. * Participates in the planning, development, and implementation of processes, practices and procedures for the site. * Respond to inquiries and concerns from the general public including municipal and housing officials, public safety officials, and other community agencies and stakeholders. Ensures responses comply with DMH and HIPPA confidentiality requirements. * Complies with all mandatory training requirements. * Maintains professional standards and utilizes opportunities for professional growth to enhance personal skills and development and to improve quality of care for individuals served. * Provides and/or arranges for transportation to ensure individual has access to needed services. * Performs other duties as assigned. * Preferred Qualifications: * Bilingual or multilingual proficiency in Cape Verdean dialects. * Strong and effective interpersonal skills, including the ability to actively listen, relate to others and demonstrate sensitivity and respect for individuals. * Proficiency in Microsoft Office applications including Outlook, Word, and Excel. * Previous case management experience. *_DMH/BMSC Mission Statement:_ * The Brockton Multi Service Center provides comprehensive services to mentally ill adults and children. Salary placement is determined by a combination of factors, including the candidate’s years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division’s Hiring Guidelines.
