Family Service Association - Fall River Clinical Care Manager in Brockton, Massachusetts

Family Service Association is seeking a full time (40 hours per week), Monday through Friday, 8:30 a.m. - 5:30 p.m., Care Coordination Supervisor in our Long Term Support Service Community Program (LTSS CP). The Care Coordination Supervisor (CCS) serves as the supervisor of the LTSS CP Supports Care Coordination team. The CCS provides regular supervision for Care Coordinators and Community Support Workers and the delivery of all LTSS CP Support Interventions.
Supervise Care Coordinators and Community Support Workers on a regular (no less than bi-weekly) basis using in group and individual settings.
Demonstrate knowledge and experience in the delivery of LTSS Supports and Care Coordination.
Ensure Care Coordinators develop person-centered care plans that reflect the voice and choice of assigned enrollees within required time frames.
Ensure LTSS CP Supports care plans are endorsed by primary care providers and shared with all members of the enrollee’s care team.
Demonstrate willingness to work with individuals with complex LTSS and BH needs.
Support the engagement of assigned enrollees within specified timeframes.
Promote achievement of Quality Measures for the LTSS CP Supports.
Support the development of Consumer Advisory Boards and provider networks.
Have a willingness and ability to work with ACOs, MCOs, state agencies, service providers and community organizations.
Willingness and ability to work flexible hours and across the geographic area served by the program.
Provide guidance to the care coordination team for the development of health and wellness coaching and current listing of community resources.
Participate in 24 hour on call coverage on a rotating basis.
Triage referrals and provide referral resource with accurate and timely availability of needed resources.
Maintain paperwork requirements for LTSS CP Supports in compliance with established timeframes.
Assist in the orientation of new employees and the ongoing professional development of all LTSS CP Supports employees.
Provide assistance to the Director in reporting and monitoring program performance.
Demonstrate and promote respectful, culturally competent and linguistically responsive delivery of services.
Understand and demonstrate commitment to agency mission, values and philosophy in performing job responsibilities, including compliance with all ethical and legal requirements of the organization.
Respond to reasonable expectations of the Director of Healthcare Transformation and the organization.
Education and Experience Required:
The ideal candidate must have a Bachelor’s degree in social work, human services, nursing, psychology, sociology or related fields.
Must have a minimum of three years of relevant professional experience.
Must have the skills necessary to engage and work with others from diverse backgrounds.
Maintain a nonjudgmental attitude toward families.
Must be willing to work across the entire geographic region assigned to the organization and demonstrate willingness to work with the target population; some local travel is required.
Must have one year experience as a supervisor.
Care Coordination, Bi-lingual capacity is desired.
Preferred Area of Educational Emphasis: RN with BH expertise, LCSW, LICSW, LMHC, LMFT, LADC 1, Licensed Psychologist, Licensed Rehabilitation Counselor, or Registered Occupational Therapist.
This position is based in the Brockton, MA area and includes travel to Fall River and surrounding areas.
