State of Massachusetts PACT Vocational Specialist in Brockton, Massachusetts

The Department of Mental Health is seeking a dynamic, recovery-oriented Rehabilitation Counselor C to serve as the Brockton Multi Service Center (BMSC) PACT Vocational Specialist. The selected candidate will serve as a member of a multidisciplinary team to provide supportive counseling, psychotherapy, and wrap around services to clients in the community. The incumbent will provide psychoeducation, skill development/vocation, social and ADL’s to the chronic mentally ill. The successful candidate will ensure appropriate medical linkage in the community and actively collaborates in the effort. The incumbent conducts on going assessments, treatment planning and discharge planning. _Please Note:_The work schedule will be(40 hours) 8:30 a.m. to 5:00 p.m. Rotating weekends (every 4thweek). The days off will rotate Week 1: Sunday/Saturday; Week 2: Sunday/Saturday; Week 3: Sunday/Thursday. Week 4: Tuesday/Saturday. _Duties and Responsibilities (these are not all inclusive_): * Demonstrates the ability to provide wrap around services for adult eligible clients by assessing their needs on an ongoing basis, by making home visits, by developing a supportive relationship, by assisting in the acquisition of entitlements, and by collaborating with other team members and agencies to ensure that clients can maximize their potential for recovery and independent living. * Serves on a multidisciplinary team and individual treatment teams by attending regularly scheduled meetings, by coming prepared to present current issues, psychosocial histories, mental status, diagnosis, treatment provided, and the treatment planned for assigned cases and by participating in discussions of other cases to ensure that clients receive optimal care. * Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals in conjunction with the individual treatment team. * Assess vocational goals and assist with preparing for, seeking, and maintaining employment and/or education. * Advocate for clients by acting as a liaison between the client and the community to ensure that clients are receiving appropriate service which may require prompt on-site community visits. * Support and teach the client in the performance of daily living tasks by assisting in the acquisition of entitlements, by navigating the legal and social service system in the community, by making home visits and by networking with other social service agencies to maximize the clients’ potential for independent living. * Provide individual and/or group readiness counseling, job skills training, benefits counseling and/or referrals, educational placements, financial aid assistance, and natural support building. * Assess and respond to crisis and emergency situations by intervening immediately with appropriate clinical treatment, intervention, contacting the on-call supervisor or contacting the Emergency Services Program. * Assumes responsibility for all administrative and clinical record documentation requirements by recording in the record, including completing Physical Health Assessments, and by submitting information in a thorough and timely manner so that other service providers are aware of treatment provided and so that the medical record is in compliance with agency and Joint Commission standards * Assumes responsibility for all internal daily and weekly PACT documentation so the team is always aware of what is needed for the client on that day. * Involves all clients and family members in the process of treatment planning and ensures that plans are agreed upon, by signature, on a quarterly basis. * Actively participates in the treatment and disposition planning for all assigned hospitalized or incarcerated clients. * Serves on committees and becomes involved in task force assignments to help share and improve service delivery at BMSC. * Act as the DMH PACT liaison for the Massachusetts Rehabilitation Commission as well as the local Clubhouse. _ Preferred Qualifications:_ * Knowledge of employment and educational systems * Familiarity with mental health diagnosis _Mission Statement:_ The Brockton Multi Service Center provides comprehensive services to mentally ill adults and children. MINIMUM ENTRANCE REQUIREMENTS: REQUIRED WORK EXPERIENCE: At least four years of full-time, or equivalent part-time, professional experience in social work, social casework, vocational counseling, employment counseling, rehabilitation counseling, or educational counseling. SUBSTITUTIONS: - A Bachelor's or higher degree in rehabilitation, vocational rehabilitation counseling, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, social work, psychology, human services, or counseling may be substituted for the required experience on the basis of two years of education for one year of experience. - A Bachelor's or higher degree in a major other than rehabilitation, vocational rehabilitation counseling, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, social work, psychology, human services, or counseling may be substituted for a maximum of one year of the required experience on the basis of four years of education for one year of experience. - A Master's or higher degree in social work, psychology, human services, rehabilitation, vocational rehabilitation counseling, or counseling may be substituted for an additional year of the required experience on the basis of two years of education for one year of experience. - Licensure as a Licensed Rehabilitation Counselor by the Board of Registration of Allied Mental Health Professionals or Certification as a Certified Rehabilitation Counselor by the Commission on Rehabilitation Counselor Certification may be substituted for the required experience. - One year of education equals 30 semester hours.
