Sign-On Bonus Eligible - Up to 8k

The Case Manager role is now eligible for up to a $8,000 sign on bonus. There is an opportunity to receive a $8,000 sign-on bonus for full-time hires working full time schedules. Important Details: Bonuses are paid out within the first 30 days of employment and are subject to applicable taxes. Full time status at BILH is considered for schedules greater than or equal to 30 hours per week; part time status is 20 to 29 hours per week. Employee must be in good standing to receive the bonus. Current BILH employees do not qualify for this bonus. This program is subject to change at any point.

Any part-time position will be pro-rated based on the full-time bonus amount.

Position Summary:

The inpatient Case Manager assigned by nursing unit coordinates discharge planning and monitors quality assurance for Lahey Clinic inpatient admissions. Leads the multidisciplinary team to ensure a timely process.

Minimum Qualifications:

Education:

• Registered Nurse, Bachelors Degree strongly preferred.Licensure, Certification, Registration:

• Must hold current Commonwealth of Massachusetts license as a Registered Nurse, and Basic Life Support (BLS)

Skills, Knowledge & Abilities:

In the absence of any of these skills, demonstrates the ability to be thoroughly trained to Proven leadership and project management experience and skill meet organizational standards.

• Effective verbal communication, problem solving and conflict resolution skills.

• Basic computer and typing skills.

• Basic knowledge of Quality Improvement techniques.

• Demonstrated ability to organize and work independently

Experience:

• A minimum of three years of medical/surgical nursing care experience. Two years of case management or utilization management experience desirable.

• Demonstrated ability to communicate effectively with medical and hospital staffs.
