One-to-one observation will be provided for patients that are deemed to be at risk of harm to self or others.

Job Description:

Essential Duties & Responsibilities (including but not limited to):

  1. Observes one or more assigned patients for harmful, dangerous or violent behaviors.

  2. Communicates with nursing staff to understand patient expectations before and during assignment. Seek clarification as needed.

  3. Remains with patient at all times unless relieved of duty by another staff member. For suicidal patients, keep the patient within eyesight and arm’s length at all times, including bathroom use. Remains quietly alert in the room when the patient sleeps.

  4. Remains alert and mentally engaged for extended periods. Uses verbal intervention to calm patient, diffuse aggressive behavior or prevent escalation of conflict.

  5. In collaboration with the nursing staff removes objects such as belts, glasses, ties, scissors and medications so that the patient may not use them to harm himself/herself.

  6. Accompanies the patient to any area for examination, x-ray, therapies, etc. Takes direction from the nurse assigned to the patient.

  7. Reports any change in the patient's behavior or condition to the nurse assigned to the patient.

  8. Remains with the patient when the patient has visitors, even if the patient requests privacy unless approved by the patient’s nurse. Notifies nursing staff of changing or unusual conditions.

  9. Completes all documents as required.

  10. Responds appropriately and immediately in emergency/escalating situations. Maintains calm manner. Trained to assist with restraints.

  11. Respects policies on patient confidentiality at all times. Receives information about the patient from the nursing staff. Directs questions to the nurse assigned to the patient.

  12. In collaboration with the clinical staff, assists patient with basic ADL’s including toileting, hygiene, etc…

Minimum Qualifications:

  1. Verbal, quantitative, and interpersonal skills, normally acquired through completion of a high school diploma.

  2. Related experience in a healthcare setting, or student in a healthcare field.

  3. CPI training preferred.

  4. Ability to remain alert for extended periods of time.

Physical Requirements & Environment:

  1. Normal patient care environment, with occasional exposure to infectious diseases, unpleasant odors, bodily fluids, and the like.

  2. Ability to walk and stand for up to 90% of work time and to push and pull heavy objects including patients and to lift 50 lbs.

  3. Adherence to proper safety precautions minimizes the risk of injury.
