Department Description:

CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.

Job Summary:

CHA PACE provides care for participants in multiple environments that include Adult Day Centers, Supportive Housing sites, and in the community. Under the direction of the Operations Supervisor and Center Director, and as a member of the PACE Interdisciplinary team, the Personal Care Attendant (PCA) provides assistance with Activities of Daily Living (ADLs) for participants in their home environment and/or as prescribed in their plan of care and daily assignment.

This PCA role provides assistance to our two Supportive Housing sites in Cambridge. The schedule for this position will include varied shifts of 6:00am - 2:30pm, or 2:00pm - 10:30pm, based on the need for care between these two sites. This schedule is provided 2 weeks in advance and includes every other weekend off.

Responsibilities:

  • Assist participants in bathing, including complete, partial or sponge in tub or shower

  • Assist or provide laundry service as assigned

  • Ensure participant home/environment is clean and tidy to promote health and safety

  • Communicate effectively with management, peers, and healthcare team members

  • Complete daily home care or day center assignment, providing support and assistance with ADLs

Qualifications:

  • High school diploma or equivalent

  • At least one year working with an older adult population required

  • Experience working as a personal care attendant, home health aide, or other direct care professional preferred
