24 hours, day/evening rotation, with an every third weekend commitment. This role will require you to be onsite in the Schrafft's Building in Charlestown MA, treating patients remotely with the flexibility of doing some in-home visits as well. Plenty of free parking!

Lahey Hospital and Medical Center is now offering up to a $3,000 in signing bonus for our Command Center Staff RN positions. Nurses must be licensed with at least one (1) year of nursing experience to be eligible for the signing bonus.

Important Details:

• Signing Bonuses are paid out in three payments. You will receive your first payment (one third of the total amount) within the first 30 days of employment and is subject to applicable taxes. You will receive the second payment at six (6) months and the third payment (final third of the total amount) owe one year following your start date and all payments subject to applicable taxes.

• Full time status at LHMC is considered for schedules greater than or equal to 30 hours per week; part time status is 20 to 29 hours per week. Roles less than Full-Time will be prorated. Please note, per diem employees are not eligible for signing bonuses.

• Current and former BILH candidates - restrictions apply. BIDMC Internal employees or employees within the BILH system are not eligible for the bonus or if you have been employed by a BILH entity within the last 12 months.

• Please note, sign on bonuses are subject to change based on the organization's hiring needs and will be determined by Talent Acquisition on an ongoing basis. BILH/LHMC Talent Acquisition reserves the right to change signing bonus eligible jobs and amounts at any time.

Job Description:

This Hospital at Home Staff RN role is for an RN who wants to be part of an innovative team working as a virtual RN in a command center. The Hospital at Home RN will be the nucleus of the care provided to inpatients who are cared for in their home by medical personnel. This RN will have virtual visits with the patient, document in the electronic medical record, coordinate the plan of care, and participate in multidisciplinary rounds. The majority of time will be spent in a central command center where you will be working with other RNs and Medical doctors to care for patients who require inpatient care, but can receive the care in their home. Position Summary: Reporting directly to the Hospital at Home Director of Nursing, The Hospital at Home Staff RN is responsible for providing high-quality patient care to various patient populations within the framework of the Hospital at Home model.

Essential Duties & Responsibilities including but not limited to:

  • Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home

  • Navigating patients and families across the continuum of care

  • Monitoring and responding to biometric data per Hospital at Home Command Center protocols

  • Conducting audio/visual patient care visits as directed by the care team and/or protocol

  • Ensuring all interventions in the patient’s care plan are implemented efficiently with a patient centered approach

  • Leveraging the resources and expertise of supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation

  • Adhering to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based care

  • Promoting professional practice and a culture of safety; willingness to engage in process improvement efforts

Clinical Roles and Responsibilities:

  • Providing protocol driven assessment, care coordination and interventions as appropriate informed by Hospital at Home protocols for diverse patient populations with a focus on oncology, medicine and post- operative care

  • Exhibiting sound clinical decision making and critical thinking skills

  • Maintain and practice in accordance with nursing guidelines per state and hospital regulations

  • Facilitating admissions to l Hospital at Home utilizing Hospital at Home guidelines

  • Serving as a role model for professional nursing practice

  • Utilizing nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problems

  • Sharing expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertise

  • Supporting research efforts to advance knowledge and promote evidence-based practice

  • Preforming other duties and responsibilities as assigned by the Senior Director of Nursing

Quality and Safety Responsibilities:

  • Ensure compliance with reporting requirements for Sentinel Events and Serious Incidents

  • Participate in quality and safety investigations and root cause analyses

  • Take appropriate action to ensure patient safety as a result of any investigation; and

  • Serve as a resource in quality improvement processes and technique

Organizational Requirements:

Minimum Qualifications:

Education:

  • Nursing Diploma or ASN, BSN preferred

Licensure, Certification, Registration:

  • A current Massachusetts RN license

  • BLS certification
