The BILH Transfer Center optimizes bed capacity across BILH facilities and its affiliates while ensuring that patients receive appropriate, high-quality care in the right setting.

The Transfer and Bed Access Nurse performs patient flow, transfer, and bed placement activities.

This is an office based position which requires strong critical thinking and clinical triage skills.

The position offers a strong connection to clinical care and multidisciplinary collaboration and communication across all system facilities.

Areas of responsibility include patient flow activities, electronic bed board operations, system coordination, quality improvement, professional growth and self-improvement, and other professional duties.

Works with tertiary hospital Admitting departments, tertiary hospital EDs, and all referring hospitals to maintain access to hospitals and assign patients to clinically appropriate beds.

Job Description:

Primary Responsibilities:

Oversees clinical and administration of patient flow and resource allocation throughout tertiary hospital inpatient areas. Anticipates, plans and implements strategies for changes in patient flow areas.

Networks with referring hospitals, providing a clinical contact for clinicians. Serves as a conduit for all transfers for all referral sources (essential)

Works with support departments, nursing personnel, case managers, and physicians to facilitate timely admissions and transfers within Emergency Departments, PACUs, ICUs, Cath Labs, Medical/Surgical Units, and Clinics. Conducts daily meetings and coordinates admissions plan. Oversees/performs the assignment of tertiary patients to units and rooms based on clinical appropriateness and resource allocation.

Participates and initiates quality improvement activities related to the patient flow throughout the health system.

In collaboration with hospital and system leadership and physician stakeholders, facilitates critical bed capacity meetings for tertiary sites.

Resolves and identifies creative or alternative plans with bed capacity constraints. Assesses actual and potential bed availability. Consistently monitors and evaluates patient census and bed availability.

Confers with physicians and nursing managers/directors to facilitate optimal patient care on accurate assessment of data. Continuous, iterative triage of patients for bed assignment requiring all levels of care, including but not limited to ICU care, transfers, and patients with complicated medical problems.

Communicates bed status between tertiary sites on an as-needed basis in order to serve incoming Transfer Volume to the local leadership. Communicates all pertinent issues to Nursing Leader.

Monitors all incoming Transfer Requests in order to determine the most appropriate level of care for the patient. In accordance with Transfer Center workflow, redirects and load balances between tertiary sites. Collaborates with leadership at community facilities for coordination of care and community level transfer and load balancing.

Interface with multi-disciplinary stakeholders to drive quality patient outcomes for high-acuity and high-need patients.

Works in compliance with Nursing Practice Guidelines which foster safe, cost effective and quality patient care.

Required Qualifications:

1. Bachelor's degree required.

2. License Registered Nurse required and Certificate 1 Basic Life Support required.

3. 3-5 years’ related work experience required.

4. Experience demonstrating professional competence and leadership.

5. Broad view of system operations.

6. Experience with computer systems required, including web-based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.
