REPORTS TO:                     Program Director, Fieldstone, Stonebrook and Vista Adult Day

POSITION SUMMARY :

The Responsible Person provides the special service and care required to each resident. The Responsible Person accommodates each resident’s care and program/activity needs , while assisting in maintaining a positive physical, nutritional and social environment for the residents. Additionally. The Responsible Person ensures each resident feels safe and purposeful. Assist team members with and contribute to the implementation of the resident service plan of care which includes activities of daily living (eating, dressing, personal care, and toileting), education, medication administration, recreation and transportation in accordance with the established policies, procedures and standards of the nursing department. The Responsible Person is directly responsible to the Program Director and Wellness Nurse. The Responsible Person maintains patient/resident privacy and confidentiality.

**This position comes with a $5k Sign-On Bonus, based on FTE

**Part Time,  16 Hours, every other weekend and every other Friday in Stonebrook, every other Tuesday in Fieldstone.  

PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS :

  • Assist residents with activities of daily living, including bathing, dressing, grooming, and bed making.

  • Demonstrate knowledge of residents in regard to background, participation needs, physical abilities, interests, cognitive capacity and attention span capability.

  • Participate in service plan updates.

  • Assist with meal service.

  • Know and comply with all resident rights.

  • Receive and give report of shift activities and concerns during shift change report by oral and/ or written log.

  • Observe and report any changes in resident's condition (behavior or health); take action to ensure safety of the resident and reports changes to Wellness Nurse or Supervisor.

  • Complete documentation accurately and in timely manner.

  • Check on resident rooms daily for general cleanliness and neatness; ensure rooms are clear of fall hazards.

  • Check common areas - including public bathrooms - for cleanliness.

  • Measure and accurately record vital signs (temperature, pulse, blood pressure) where appropriate, as per state regulations.

  • Assist residents with medication administration. Participate in narcotic count.

  • Enter resident information under progress notes in Sigma Care.

  • Communicate with physicians, nurse practitioners and pharmacy as needed for service coordination.

  • Report resident and family concerns/issues to Director or Wellness Nurse.

  • Recognize and assist in the prevention of elder abuse, neglect, and exploitation (financial and other).

  • Comply with health care instructions explained by health care professionals and promptly and appropriately seek and obtain medical attention for residents when needed.

  • Respond to emergencies and personal emergency response system.

  • React calmly and effectively in emergency situations.

  • Assure resident information is communicated to the team member’s and thoroughly reported in the communication log (i.e. appointments, outings, etc.).

  • Document residents' material needs (i.e. briefs, clothing, toothpaste, pads, etc.) and inform family.

  • Accurately complete incident reports.

  • Complete annual in- services.

  • Attend team member meetings on a regular basis.

    Responsible Person on cart responsibilities:

  • Orient new Responsible Person team members to the work environment.

  • Communicate resident issues or team members concerns to Wellness Nurse.

  • Report all med errors, falls, hospital returns, and incidents to Wellness Nurse.

  • Communicate with families regarding resident updates, medical emergencies and resident needs.

  • Take the lead in emergency situations, reacting calmly and efficiently, notifying appropriate parties and ensuring documentation is completed.

  • Ensure procedure is followed for residents returning from hospital or rehab.

  • Act as a role model for peer team members in demeanor, appearance and work standards, and remain professional in all communication.

  • Ensure all documentation and duties for shift are completed before end of shift.

  • Ensure incident reports are filled out accurately and thoroughly and appropriate parties have been contacted. (Call Wellness Nurse to report any incidents).

  • Make copies of forms, assignment sheets as needed.

  • Order medications from pharmacy as needed.

  • Attend required in-services/meetings and completes Relias modules monthly.

    QUALIFICATIONS:

  • Certified Nursing Assistant (CNA) with current/active certification - required

  • One year Long Term Care experience - preferred

  • RP on cart must pass medication test with 85% or better score to obtain competency - required

  • Good communication and interpersonal skills with team members, residents.

  • Good computer and technology skills.

  • Must possess the ability to make independent decisions when circumstances warrant such action.

  • Must possess the ability to deal tactfully with team members, patients/residents, family members, visitors, government agencies and the general public.

  • Must be able to work harmoniously with all team members

  • Must possess patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult patients/residents.

  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.

  • Must not pose a direct threat to the health or safety of the other individuals in the workplace.

    PHYSICAL REQUIREMENTS/WORKING CONDITIONS :

    Primary work will be performed indoors in an office setting, may be exposed to the elements (heat, cold, rain, snow); walking and standing frequently; occasional lifting of up to 45 pounds.

  • Work indoors in a climate controlled setting.

  • Possible exposure to unpleasant odors.

  • Possible exposure to chemicals as identified in the MSDS Manual.

  • Possible exposure to infections, diseases and blood borne pathogens.

  • Possible exposure to residents who may be ill, confused, irritable and irrational.

  • May be subject to angered and/or emotionally upset family members, personnel and visitors.

  • May work beyond normal working hours and on weekends and holidays when necessary.
