POSITION SUMMARY :
The Transition Care Liaison is a team player with sales and marketing experience and solid relationships in the healthcare field. A Transition Care Liaison must be persuasive, flexible, driven, well-mannered and accurate. A Transition Care Liaison must have the ability to interact with all customers, both internal and external with the professional and diplomacy necessary to portray The Overlook in a positive manner. Must possess superlative communication and interpersonal skills across all levels while maintaining sensitivity to resident/patient needs. A Transition Care Liaison must represent The Overlook at appropriate places, events and functions and act as a liaison between all Overlook services and its accounts.
The Transition Care Liaison is responsible for generating client referrals from professional referral sources for The Overlook’s Home Health and Hospice Services. The ideal candidate must have 3-5 years of experience in sales in marketing focusing on Home Health and Hospice with a proven track record of success and achieving agency/organizational referral goals, and a skillset for developing solid referral relationships in the healthcare field. The Transition Care Liaison is responsible for meeting and or exceeding specific referral and census goals.
PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS :

Acts as a key player in the overall vision for Home Health and Hospice Services at The Overlook.
Market Intelligence and Sales Planning:
Gathers and uses market data to maximize potential referral sources and increase sales
Follows future trends in territory and responds preemptively to market changes
Maintains existing and expands new customer base by developing relationships with the following key stakeholders:
Hospital Case Managers/Discharge Planners
Directors and nursing personnel from area clinics and nursing homes
Physicians
Insurers and case management firms
Elder Law Attorneys
Community Agencies
Conducts extensive research on assigned territory and develops and implements plans and goals to cultivate new business with key stakeholder groups and nurture existing accounts, in pursuit of meeting weekly, monthly and annual referral goals.
Identifies and overcomes barriers to referrals and addresses any obstacles to achieving results.
Develops and presents a territory/ sales plan for assigned accounts.
Expands customer base by constantly identifying potential new accounts.
Meets or exceeds weekly, monthly and quarterly goals established with manager.
Activity and Activity Management:
Pre-plans weekly marketing activities in accordance with a monthly activity/marketing plan to ensure all goals are met.
Performs regularly scheduled sales calls to reach monthly goals
Keeps accurate records and logs of all sales activity in accordance statistical and regulatory requirements and company process/procedures/tools.
Manages and coordinates referrals from providers, facilities or any other referral source.
Coordinates and attends related trade shows, conferences and events, as approved by manager.
Other Responsibilities:
Maintains and monitors spending in regards to expense accounts.
Attends external meetings as needed.
Rotates holiday coverage as requested.
Adheres to OVNA and MHS policies and procedures.
Participates in quality management and safety activities, as needed.

QUALIFICATIONS :

Educational Requirements and Experience:
Post High School education - required
A Bachelor’s degree in business, public relations/marketing or health care related field ) - preferred
Experience in Home Health, Hospice and Long-Term Care screening and/or sales experience - preferred
Minimum of three to five years in Sales and Marketing experience working in a health care field - required
Valid MA driver’s license and reliable transportation - required
PHYSICAL REQUIREMENTS/WORKING CONDITIONS :
Works primarily indoors in a climate controlled setting.
Possible exposure to unpleasant odors.
Possible exposure to chemicals as identified in the MSDS Manual.
Must be able to exhibit emotional intelligence and have experience working in difficult settings and environments.
May work beyond normal working hours and on weekends and holidays when necessary.
Required to travel to multiple work sites and up to 200 miles per day.
