Job Description Position Title: Speech Therapist

REPORTS TO:         Manager of Clinical Practice, Home Health     

SUPERVISES:           N/A   

DEPARTMENT:      OVNA Home Health

COMPANY:               Overlook Home Health and Hospice

POSITION SUMMARY :

The Speech Therapist provides skilled speech therapy services in home and skilled nursing settings consistent with ASHA professional standards and MHS/OCAH policies. Participants in the development of programs necessary to achieve objectives as they relate to patient care.

PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS :

  • Completes all documentation required and in accordance with state and federal regulations and policy.

  • Perform patient assessments and evaluations to determine functional level and establish a therapeutic program.

  • Develop a therapy plan of care with the overall plan of care to include goals and discharge plans.

  • Document, within time frames, observations, patient’s response to treatment, treatment programs, degree of progress and notable changes in patient condition.

  • Communicate and coordinates patient care with other members of the patient’s health care team.

  • Provide patient caregiver, and family teaching related to the patient’s speech therapy program and its maintenance.

  • Provide speech therapy in accordance with physician’s orders and accepted professional practices.

  • Assist the patient and/or family caregiver to procure needed equipment or supplies.

  • Utilize appropriate hand washing, standard precautions and personal protective equipment.

  • Attend team member meetings and patient conferences. Provides proof of attendance at mandatory or training programs.

  • Other duties as necessary.

    QUALIFICATIONS :

    Educational Requirements and Experience:

  • Graduate of an accredited Speech Language Pathology program

  • Current license to practice as a Speech Language Pathologist in the state of Massachusetts

  • Must possess a current Driver’s License and reliable transportation

    PHYSICAL REQUIREMENTS/WORKING CONDITIONS :

    Must provide proof of physical with no limitations related to the Principle Job Duties and Essential Functions as outlined above. This includes but is not limited to standing, walking, sitting use of fine motor skills, no restriction on upper or lower range of motion or lifting ability to stoop, kneel, crouch or crawl. The ability to speak, hear, taste, smell. Unimpaired vision at distances both near and far. No restrictions on peripheral vision, depth perception, and ability to adjust focus. May be exposed to blood borne pathogens. May be subject to emotionally upset residents, family members, personnel or visitors. Possible exposure to the outdoors, unpleasant odors, house hold chemicals, and ill patients.

    Must be able to travel to multiple work sites with in the work day using personal, fully insured vehicle.

  • Possible exposure to unpleasant odors.

  • Possible exposure to chemicals as identified in the MSDS Manual.

  • Possible exposure to infections, diseases and blood borne pathogens.

  • Possible exposure to residents who may be ill, confused, irritable and irrational.

  • May be subject to angered and/or emotionally upset family members, personnel and visitors.

  • May work beyond normal working hours and on weekends and holidays when necessary.

The organization embraces its rich history and heritage, owning and managing 218 Independent Living Apartment Homes and Cottages, 28 Enhanced Care Private Suites, a 14- Suite Assisted Living Memory Care neighborhood, 27-unit Short-term Post-Acute Rehabilitation, 112-bed Long Term Care Skilled Nursing, as well as Home Health, Hospice, Palliative Care, Private Duty and Care Management divisions.
