State of Massachusetts Veteran Care Coordinator-RN IV in Chelsea, Massachusetts
The Massachusetts Veterans Homes are independent state agencies that organizationally report to the Secretary of the Executive Office of Veterans Services (EOVS). The Veterans Homes are owned and operated by the Commonwealth of Massachusetts and address the needs of a wide variety of veterans, including long term care and domiciliary services meeting the veteran residents’ care and psychosocial needs. The care and services provided by the Veterans’ Homes are supported by a staff of approximately 450; representing physicians, advanced practice clinicians, nurses, aides, social workers, recreation therapist, physical and occupational therapy, speech language pathology, educators, building trades, dietary, housekeeping, managers, support staff, consultants and contractors. GENERALSTATEMENTOFDUTIESANDRESPONSIBILITES Responsible for the overall management of designated Veteran Care Center with performance measured in three major areas:Veteran care management,Veteran environmentmanagementand personnelmanagement.Assumes responsibilityformaintaining thestandards ofnursing careandpractice, policies and procedures with authority and responsibility on a 24-hour basis.Delivers general age-specific nursing care for adult and geriatric Veterans,predominantly male;with varying degrees of chronic physical disabilities and diseases;as well as psychiatric diagnoses, depression, and various dementia related disorders, toincludeAlzheimer's disease;with accompanying behavioral issues.Provides guidance and supervision of assigned staff of the Veteran Care Center. DETAILEDSTATEMENTOFDUTIESANDRESPONSIBILITIES VeteranCareManagement · Assesses, plans, andevaluates nursing care andVeteran progress towards goals, directing staff as needed. · Responsibilityforensuring thatthe Veteran's Resident Assessment Instrument (RAI) process, MOS/RAPS inclusive is appropriate and timely. · Participates ininterdisciplinaryteam process tointegrate nursing into acoordinated planof care with all other support services. b.AssuresthatVeterancaregoalsare mutuallyestablishedwithVeterans,families&staff. · Maintainsowncompetenceand actsasclinical resourceandexpertrolemodeltostaff. · Holdsstaffaccountableforown clinicalpractice,includinguseofnursingprocessand appropriatedocumentationtomeetallregulatory body standards. · Assures staff knowledge andintegration ofage specific nursing of the Veteran population as a critical element in the assessment, planning, implementation, and evaluation of care. · Develops systems forthe evaluation ofVeteran caredelivered inrelationtotheVeteran carerequired, such as on-going medical record review, report findings, actions, and follow-up through the Nursing Department Performance Improvementprocess. · Utilizesappropriate resources withinfacility toidentifyandprovide education, technical orinformation services needed to staff or Veterans. · InteractswithappropriatemanagementpersonnelregardingVeterancareandrelatedissues. · Member ofappropriate clinicalcommittees tosupport Veterancare. Veteran Environment Management · Developsacarecenter'sphilosophy/goalswhichreflect theuniquenessofthecarecenter,thatisconsistentwiththephilosophyofboththenursingdepartmentandtheSoldiers'HomeinHolyoke. · Managesthecarecenterfiscallybycollaborating with theDirector of Nursingmaintaining accountabilityforapproved personnelsupplies and expensebudget. · MaintainsasafeenvironmentforVeterans,staffmembersandvisitors. · Works withother ancillary support servicesinthecarecenter,initiatingappropriate communication and planning toensure a constantlevel of services to meeting Veteran care needs. * Prepares written reports oncare centerissues,trends, andprogress towards goals. PersonnelManagement · Participates ininterview process fornewemployees; assists newstaffmembers inorientation process, completes a competency-based skillsinventoryand assigns senior staff to precept orientees. · Promotesteamworkwithallstaffassignedtoshift. · MonitorsactivitiesoftheVeteranCarestaffforadherencetostandardsandneedforeducation. · Promotes communication to staff members byconducting regular staff meetings toshareinformation; maintains active communicationby use of logs, etc. to personnel on the Veteran care center. · Promotesdevelopmentofallstaffmembersby: a. meeting witheach staff member onregular basistoevaluate performance,identifyareasfor growth, set mutual goals and provide progressive work assignments. b. develops leadership skillsinpersonnel byconducting/preceptinginorientation ofnewpersonnel and guiding the staff in the development ofleadershipabilities. · Conducts progressivedisciplinewithindividualstaffmembersasneeded,documentingappropriately. · Develops a Veteran Care Center based schedule and collaborates with other nurse coordinators/supervisorsto assure balanced staffing patterns.Monitors use of sicktime ofstaff. · Evaluates eachmemberutilizingEPRS system. FacilityManagement. · Functions as facility supervisor onweekends andholidays ona rotating basiswithotherVeteran CareCoordinators. · Relievesother VeteranCareCoordinators,onasharedbasis,duringvacation,extendedillness,etc. · Identifies goals forprofessional development andattendscontinuing education opportunities toattaingoals. · MemberofotherSHHfacilitycommittees * Performsotherrelateddutiesasrequired. QUALIFICATIONSREQUIRED ATHIRE * Knowledge of principles and age-specific practices of nursing * Knowledge of standardnursing policies, procedures, methods, andtechniques,includingsafeadministration of medication. * Knowledgeofemergencynursingprocedures. * Knowledge ofprinciples andpractices ofhealthcarepractices andmethods in evaluating nursing practice. Knowledge of the coordination of the interdisciplinary health care team utilizing the Resident Assessment Instrument (RAI) process. * Ability to maintainconfidentialityofVeteranmedicalinformation. * Knowledge ofResidentAssessmentInstrument (RAI),includingMOS/Raps process andinterdisciplinaryteamapproachto careplanning. * Ability tosupervise, coachhealthcarepersonnel utilizing soundpersonnel practices including counseling and progressive disciplinary action skills. * Intravenousskillsandknowledge. * Ability toconduct staffinterviews andknowledge ofhiringpractices. MINIMUM ENTRANCE REQUIREMENTS: Applicants must have at least (A) two years of full-time, or equivalent part-time, experience as a registered nurse in a recognized hospital, clinic or medical facility and (B) of which at least one year must have been in a supervisory, administrative or managerial capacity or (C) any equivalent combination of the required experience and the substitutions below. Substitutions: I. A Graduate degree with a major in Nursing may be substituted for a maximum of one year of the required (A) experience.* No substitutions will be allowed for the required (B) experience. Education toward such a degree will be prorated on the basis of the proportion of the requirements actually completed. SPECIAL REQUIREMENTS FOR ALL LEVELS INSERIES: Currentand valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing and CPR certification. *Recent graduates from an approved school of nursing must have applied for the first available registered nurse examination.
