Family & Community Integration Specialist - AO/DYS Youth/Young Adult Residential Chicopee, MA ID: 674-379 Full-Time/Regular Family & Community Integration Specialist (Family Worker) If you love working with children and families, this role is for you. Oftentimes, the Family & Community Integration Specialist is referred to as the "face and voice" of the program. This Family Worker will be based in our Alternative Options, Chicopee location. Duties include: Gather info from parents and other appropriate collaterals. Supply proper info of our program including the layout of what their child's time here may look like. Be able to complete an intake for incoming youth Set up weekly visits with parents and youth and monitor visits Organize a family engagement event once monthly. Checking in with parent before/after visits/phone calls Help identify/assign advocates to the youth. Creating advocate reports weekly, or as needed for the advocate to complete with their assigned youth. Work alongside clinicians to determine goals, life skills, etc Assist staff on the unit when needed Developing a rapport with youth while also maintaining appropriate boundaries Conducting parent surveys when a youth has completed their time at our program Coordinate home, ground, and community passes using the DYS pass policy Other duties will be assigned upon being hired Collect specific data for Clinical Director (restraint data, Learning Experience tracking, etc.) Required Skills Family & Community Integration Specialist Qualifications: Bachelor's Degree in Human Service, Social work. Minimum of 2 years experience in Human Service field working with troubled adolescents. Ability to work with a team as well as independently. Must hold a valid Driver?s License and a vehicle available for day to day use on job. Must meet agency requirements for driving history record, and Background Checks. Demonstrate excellent verbal and written skills. Spanish speaking preferred but not required.
