State of Massachusetts Human Services Coordinator I in Danvers, Massachusetts

_This position is located in the Hogan Regional Center in Danvers, MA_ The Massachusetts Department of Developmental Services (DDS) is dedicated to creating, in partnership with others, innovative and genuine opportunities for individuals with intellectual disabilities to participate fully and meaningfully in, and contribute to, their communities as valued members. The Department of Developmental Services (DDS), Northeast Regional Office is seeking candidates for the position of a Part-Time Human Services Coordinator I (HSC I). The Human Service Coordinator I is the primary contact for individuals and families served by the Department of Developmental Services and are expected to effectively represent the agency by building respectful and collaborative partnerships with individuals, families, guardians, provider agencies and other stakeholder groups involving the work of the Department. Duties and Responsibilities(these duties are a general summary and not all inclusive): * Follow up in a timely manner in all inquiries/applications concerning eligibility criteria for DDS services and educate individuals, families and service providers, as assigned. * Arrange for intake appointment with applicants for DDS services and conduct intake interviews at a location convenient for the applicant. * Obtaining all historical information relevant for assessing an applicant’s eligibility for DDS services, including but not limited to: medical and psychiatric assessments, psychological assessments, school records, human services agency records and information from all previous and current service providers. * Organize and summarize intake information in a standardized format consistent with procedures outline in the Department’s Eligibility manual/workflow. * Make structured observations and gather information regarding adaptive behavior skills and deficits of applicants under the guidance if the Regional Eligibility Team Psychologist. * Participate as a member of the Regional Eligibility Team in analyzing data relevant to the determination of eligibility for DDS services. * Enter and maintain data in appropriate databases/record keeping systems in an accurate and timely manner. * Responsible for writing reports and analyses of information, as required and to preform other related duties as needed. * Provides information and referral services to applicants, families and guardians during the period between initial contact and formal eligibility determination, including referral to appropriate Area Offices for details information about local DDS and generic resources. * Maintain liaison with various public service and private agencies, organizations and officials. * Ability to exercise discretion in the handling of confidential information. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.
