Eliot Community Human Services Stabilization Specialist (Part-time weekends) in Danvers, Massachusetts

Responsibilities:
Provide supportive stabilization services to clients in need of crisis supports and community stabilization at the CCS program
Provide side by side interventions to individuals/families that focus on problem solving, skills training, modeling behavior, optimism and encouragement
Provide interventions at CCS based on client preference, client need, and at the clinical direction of the triage/urgent/CCS care team
Provide direct support to clients focused crisis interventions, stabilization, planning and evaluations through the treatment planning process at CCS
Assist clients at CCS in ADLs both related to maintaining their space, personal hygiene and developing ADLs plans for the community
Direct support and interventions related to short-term treatment planning, stabilization and urgent care at CCS that include evidence-based practices supported by Eliot
Complete NOMS assessments as assigned, aspects of clinical assessments, and other documentation and required to meet CCS/Eliot standards
Provide short term case management supports to individuals including: managing income, insurance, other entitlements, accessing other services and community resources and scheduling/coordination of all medical and psychiatric appointments
Support warm handoffs from CCS to triage, stabilization and urgent care to CBHC teams
Qualifications:
Requires a Bachelor’s Degree in a Human Services field and one year of experience with adults, families or children with severe and persistent mental illness and/or emotional disturbances
Must have a strong commitment to recovery-oriented services.
Must have skills to establish supportive relationships with persons with severe and persistent mental illnesses, kids, families and respect for their preferences for treatment
Must have ability to communicate clearly and effectively with individuals in rehabilitation and treatment. Must have ability to provide flexible services that are community based and focused on team work
Compensation: $23.08/hour
Schedule: Saturday 3pm-11pm and Sunday 7am-3pm
