Position Summary:
The CDTM Clinical Pharmacist is part of the clinical pharmacy team, whose primary responsibility is providing collaborative drug therapy management services (CDTM) within our primary care departments, and to optimize medication-use by providing direct patient care, provider support, drug information and consultation, and patient and clinician/staff education about medications.

The ideal candidate will be a curious, organized, motivated, detail-oriented, and self-driven clinical pharmacist with extensive experience in direct patient care managing chronic conditions such as diabetes and high blood pressure. Strong written and oral communication skills and preference for Spanish speaking will be integral to communicating daily with patients, providers, and the pharmacy team. A desire to be part of an innovative team working to improve patient and providers medication use experiences is shown in their passion for advocating for highest quality pharmacy care for the patient.
Responsibilities Include:Patient Care
Provide evidence-based Collaborative Drug Therapy Management services in the primary care setting through collaborative practice agreements and practice specific protocols that will address the identified needs of the patient
Assess and monitor the drug therapy needs of patients through consultations, patient education, and clinical laboratory monitoring
Assess patients for medication adherence barriers and develop a plan to maximize patient’s medication success through improving or maintaining adherence to medications
Provide immunizations to patients based on ACIP recommendations and health center standing orders
Provider Support
Serve as expert in providing drug information services including optimal dosing, contraindications, interactions, pharmacokinetics and pharmacodynamics
Participate in the development of organization-wide drug therapy management materials, tools, and workflows
Develop and deliver provider drug education seminars to promote safe, effective, and financially responsible prescribing
Pharmacy Program Support
Assist in the development and execution of clinical pharmacy programs and initiatives
Optimize medication use through participation in medication related committees, workgroups, and quality improvement initiatives
May serve as preceptor for pharmacy students
Guide and report on 340B processes and utilization
Serve as a liaison between the dispensing pharmacy and clinical activities to support overall pharmacy program
Qualifications:Required
Graduate of an accredited college/university with B.S. in Pharmacy or Pharm.D. degree
Current active pharmacy license in good standing from the Massachusetts Board of Registration in Pharmacy
At least five years of pharmacy experience or completion of an accredited 1-year clinical pharmacy residency with an additional 2 years of experience as a practicing pharmacist; or having completed 12 months of CDTM practice under the direction of a practicing CDTM pharmacist during the residency (preferably with experience in diabetes and hypertension management)
Must meet or be eligible to meet all criteria for CDTM per Massachusetts Board of Registration in Pharmacy regulations
Proof of training and successful completion of “ACPE” accredited training to administer vaccines that meets Massachusetts Board of Registration in Pharmacy
Strong interpersonal, presentation, oral and written communication skills
Self-driven with the ability to recognize areas of potential program growth
Preferred
Fluency in a Spanish strongly preferred
Board certification (Pharmacotherapy (US-BCPS) or Ambulatory Care (US-BCACP))
Professional certifications including MTM, Diabetes
Knowledge of health plan drug formularies, 340B pharmacy program & medication savings programs
Experience with Continuous Glucose Monitoring (CGM)
Experience with Epic electronic medical record
Computer proficiency in Microsoft Office
Payrate: Starting at $127,963.00, and up to $204,740 annually, based on experience
