American Renal Associates Home Training Registered Nurse in Fall River, Massachusetts
Summary/Objective The Home Training Registered Nurse evaluates and trains the dialysis facility?s home patients and families for the home dialysis program. This position exercises knowledge of patient rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all patients in a safe, secure environment. Position must demonstrate familiarity with dialysis procedures and the knowledge, skills and judgement necessary to assume the responsibility of caring for the dialysis patient. Essential Functions 1. Complies with all American Renal Management, LLC (ARM) and Innovative Renal Care (IRC) policies and procedures applicable to the position as well as federal, state, OSHA and other agency regulations. Maintains HIPAA compliance. 2. Promotes positive public relations with patients, their family members, guests and co-workers. Uses tactful and appropriate communications in all work related interactions. 3. Maintains relationships with and contacts local management from vendor companies for assistance and problem solving on equipment and education. 4. Works closely with the Medical Director and Clinic Manager to ensure the best quality of care for the patients. Informs physician in a timely manner of any unusual findings. Escalates issues as appropriate regarding reported complaints, problems and concerns. 5. Evaluates and trains patients, plans and organizes teaching and training classes, orders or assists in ordering patient supplies, and provides surveillance of the patient?s home adaptation by: a. Providing visits to the patient?s home b. Implementing requirements for home administration of medications c. Managing patient problems and emergencies d. Reviewing lab work, assessing additional training needs, and completing all documentation and training certificates as needed. 6. Coordinates patient care with clinic visits to encompass visits with the Interdisciplinary Team (IDT). 7. Manages the ordering of equipment, inventory, serial number logs and maintenance requirements. 8. Completes requirements for in-service training. Assists with patient and staff orientation, education and in-service programs. 9. As a member of the Interdisciplinary Team (IDT), participates in the completion of Comprehensive Assessments and Plans of Care for all patients, including participating in care plan meetings and ensuring coordination with other service providers as needed. 10. As a member of the Total Quality Management (TQM) team, participates in the facility?s Continuous Quality Improvement process. Attends meetings, monitors outcomes including peritonitis rates and PET test results, reports on goals not met, discusses incident reports and executes action plans in accordance with the requirements of the TQM Program. 11. Performs job duties in accordance with all state/licensing requirements. 12. Performs additional duties as requested by the Medical Director and Clinic Manager.
