Family Service Association - Fall River Licensed Practical Nurse Per Diem in Fall River, Massachusetts
Our Countryside, Gen Center and Cornerstone Adult Day Health Centers in Fall River and Raynham, MA have an exciting Per Diem Float opportunity for a Licensed Practical Nurse (LPN). The hours are 8:00 a.m. - 4:00 p.m. Monday through Friday; with no weekend or no major holiday shifts.
Job Description:
Assist Program RN as liaison between physician and participants for medical information and service authorization of participants.
Complete medical assessment forms as designated by RN.
Monitor participant’s health status.
Provide case management services as designated by RN.
Assist Program RN in reviewing and supervising participant’s personal care services.
Attend monthly supervisory meetings with RN.
Assist RN with other services involving health issues of participants.
Assist RN in writing monthly nursing progress notes in each participant’s record.
Assist the Program Coordinator with the implementation of policies and procedures.
Ability to provide services outlined in this job description at any of the three Adult Day Health sites as assigned by the Program Coordinator.
Respond to reasonable expectation of the Agency and/or supervisor.
Education and Requirements:
Current licensure in the Commonwealth of Massachusetts Board of Registration in good standing,
CPR certification.
Two years of experience working with aging and disabled population required; previous experience working with mental health, dementia, veterans', dual diagnosis preferred.
