Family Service Association - Fall River Social Worker in Fall River, Massachusetts
We are offering a $500 sign on bonus!!
Our Cornerstone Adult Day Health Program in Fall River, MA is seeking a part time (21 hours per week) Social Worker to provide case management services and monitor mental health status to program participants. The hours are Monday through Friday, 10:00 a.m. - 2:00 p.m.
Duties and Responsibilities :
Provide case management services
Develop and lead groups for the participant.
Assist the program coordinator in referral, admission and discharge process.
Write progress notes quarterly.
Inform participants and their families/caregivers of available community services, and make referrals as indications warrant.
Attend monthly supervisor meetings with the Program Coordinator (Bi-weekly with LICSW if not licensed).
Assist, as necessary, in the delivery of other required program services
Participate in the bi-annual or quarterly review of each participant’s care plan as well as assisting with the development of care plans.
Assist Program Coordinator with the implementation of policies and procedures.
Assist Program Coordinator with record reviews, documentation, compliance, and implementation of policy and procedure.
Ability to provide services outlined in this job description at any of the three Adult Day Health sites as assigned by the Program Coordinator.
Respond to reasonable expectations of the Agency and/or supervisor.
Education and Experience Requirements:
Bachelors Degree required.
At least One (1) year of recent social work experience working with disabled/elderly adults in a professional capacity is required.
