Admissions/Discharge Nurse, RN for Fitchburg Healthcare

The A.D.N. will be responsible for overseeing and completing the required assessments and documentation for the effective management of residents and will monitor residents to ensure targeted discharge, education and home care needs are met while working with social services, direct care staff, and nursing leadership.
Supervises the clinical care planning process to ensure the baseline care plans are initiated and coordinate the delivery of quality resident care.
Ensures that a copy of the baseline care plan is shared with the resident and resident healthcare representative, if applicable.
Collaborates with physician(s)/nurse practitioner(s) to ensure that reconciliation of medication is completed to facility the continuum of care.
Oversees the collaboration process to meet the critical needs of each resident.
Reviews all admission orders for accuracy.
Utilizes best practice guidelines.
Manages the delivery of resident care, maximizing quality and efficiency.
Provides assessment for resident upon admission and periodically as regulations or changes in condition requires.
Obtains and implements physician orders. Ensures documentation is current and accurate.
Provides education and instruction to residents, families and care givers as required.
Analyzes problems, plans resolution strategies, implement plans and evaluates outcomes.
Participate in 72 hour meetings, care plan meetings, skilled meeting, Medicare meeting Interdisciplinary Team Process:
Participate in family conferences to discuss the resident care plan if indicated or required.
Facilitate discharge planning.
Communicate any significant clinical changes in the resident that would alter the care plan.
Identify the need for health teaching, teach nursing care issues to the resident and/or family members and verify/document understanding in a timely manner.
Completes discharge training/education for residents/families under the Bundle Program.
Maintains and updates binders on rehabilitative units up to date so all staff are aware of the Bundle Program for the Resident and goals for discharge.
Completes and facilitates the completion of required documentation, i.e., admission assessments, risk assessments, discharge teaching, discharge paperwork and CMA Bundled Payments for Care Improvement (BPCI) documentation.
Initiates the development and implementation of resident care standards, policies and procedures to be in compliance with regulatory requirements. .
Ensures effective communication with the client/resident, family, and interdisciplinary team.
Supports facility management and staff by facilitating change in a positive manner.
Ensures a safe and secure environment for residents, staff, and visitors.
Shares current knowledge and up-to-date clinical expertise with staff through formal and informal education. Quality Assurance:
Degree from an accredited school of nursing, college or university for Registered Professional Nurses.
Current MA license and registration as an RN.
Minimum of 1-2 years of recent acute/long-term care management preferred.
Experience in team structured environment.
Solid assessment and care planning skills.
Proficiency in the care planning process.
Outstanding collaboration and communication skills, verbally and in writing.
