The Devereux CARES (Center for Autism Research and Education Services) School in Fitchburg, MA is a small private special education school that supports students ages 8- 22 with Autism Spectrum Disorders, developmental disabilities, dual diagnoses, and social skills deficits. Our primary goal is to teach functional and pro-social skills that can be easily generalized to the student’s community and home. There is a strong emphasis on functional communication and skill acquisition using Applied Behavior Analysis and Positive Behavioral Interventions and Support. At The CARES School, students learn academic, social, daily living, and independent living skills, as well as community and vocational skill development.

The Case Coordinator serves as liaison between center, families, agencies, social workers, lawyers and otherparties. Gathers clinical information and completes documentation as assigned. Serves as a member of individual treatment teams and community milieu team. Other duties of the position include:

-Assisting with adult transition paperwork and processes

-Establishing and maintaining contact with guardians and funders

-Set up school placement

-Communicate with providers (i.e. schools, outpatient providers)

-Notification of significant incidents

-Completion of Youth Readiness Tool

-Scheduling and facilitating treatment meetings

-Initiating applications for adult services

-Contact notes

-2 nd level RADAR approval

-Discharge planning

-Provide data for and attend milieu meeting

-Seek consent for any external services or activities

-Maintaining green books

-Maintaining Outlook calendars

-Additional transportation as needed

-Attendance at case reviews

Qualifications

EDUCATION : BS/BA in social work, sociology, psychology or related field required. Proficiency in oral and written communications as a minimum requirement.

EXPERIENCE :Minimum one year experience working with children/adolescents with mental health disabilities or graduate study in counseling, casework, social work, or related field.
