Job Summary:

The Part-time MDS Coordinator will work 3 days per week. The ideal candidate is experienced with all areas of MDS and will work as part of a team to complete OBRA and PPS assessments.

Essential Duties and Responsibilities:

Schedules MDS assessments per guidelines, as assigned and maintains a monthly calendar. Informs members of the Interdisciplinary Team with the ARD and MDS’s due. Schedules Care plan meetings as needed and notifies Interdisciplinary Team Members of schedule. Notify members of the IDT of changes or additions to monthly Care Plan Meeting schedule, for new admission, change of condition or interim meetings. Contribute to the Interdisciplinary care plan by communicating any resident issues that are pertinent to the plan of care. Review charts and complete all MDS for assigned residents. Conduct interviews as assigned, with residents and staff (according to MDS guidelines) to ensure that the information captured is accurate and timely. Identify and communicate need for Significant Change in status assessments. Complete MDS/CAAs timely per schedule. Electronically submit MDS information. Review, revise and correct, and resubmit as appropriate. Complete Discharge Assessment and entry/re-entry tracking forms per Federal regulations. Audit MDS assessments and CAAs from all disciplines and notify discipline of any discrepancies. Assist with necessary completion of the nursing portion of the PPS MDS/CAAs in the absence of the Medicare MDS Coordinator as needed. Participate in Medicare meetings in the absence of the MDS Coordinator as needed. Review the ADL flow sheets and communicate with nursing and C.N.A.’s when discrepancies are found and ensure that corrections are in place. In collaboration with the Medicare MDS Coordinator and Staff Development, orient and train staff to the RAI and care planning process to assure compliance with State and Federal Regulations. Maintains a viable working relationship with all disciplines involved in the planning of resident care. Participates in facility QA initiatives, including printing QM report monthly and participating in subcommittees for follow up investigation procedures of identified areas of concern. Manages the Case Mix Index by facilitating the weekly Case Mix Meeting, reviewing case mix reports, attending daily clinical meeting, choosing ARD’s and scheduling MDS’s accordingly if applicable. Education/Experience:

Graduate of an accredited school of Nursing and current MA License required. BSN preferred. Five years’ experience in geriatric nursing required. Two years’ experience as an MDS Nurse and AANAC certification preferred. Must be well-informed about long term care reimbursement and have knowledge and training in all aspects of the scheduling and completion of the MDS and CAA processes. Pertinent job-related computer skills required. SNF Medicare experience preferred. Must possess the ability to deal tactfully with staff members, residents, family members, visitors, government agencies and the general public.
