Community Health Connections (CHC) is a multi-site, non-profit health care center offering urgent care, primary family medical and pediatric care, preventative and restorative dental care, oral surgery, behavioral health services for children and adults, and substance use disorder treatment, and specialty services including optometry eye care, optical shop, acupuncture, nutrition consultations and podiatry. CHC is mission-driven, providing compassionate, quality health care regardless of income or health insurance status. CHC has five sites within Fitchburg, Gardner and Leominster with decades of experience as a Federally Qualified Health Center (FQHC), serving 36 communities in North Central Massachusetts.
Under the general supervision of Site Manager or Manager of Clinical Office Staff, the Referral Assistant processes referrals and facilitates the communication of patient related information.
Responsibilities include:
Processes medical related referrals with outside organizations and specialists in the time frame requested by the medical provider.
Obtains necessary prior authorizations to book specialty appointments and documents in the order.
Schedules appointments with outside providers and/or faxes required information to schedule the appointment.
Follows up on appointments booked to ensure office receives documentation from the visit and “closes the loop.”
Communicates with providers as needed to ensure timely processing of referrals.
Attends meetings as required by health center.
Adheres to HIPAA regulations when communicating patient medical PHI.
Minimum Qualifications:
High School Diploma or GED required, Associate Degree in Medical Office Assistant or equivalent preferred.
One (1) year similar experience preferred.
Work experience similar to a medical office environment preferred.
Computer skills for accurate data entry preferred.
Knowledge of basic medical terminology required.
Demonstrated interpersonal relationship skills required.
Demonstrated proficiency in reading, writing and speaking in English required.
Demonstrated ability to work in a fast paced medical office environment preferred.
