Looking for a Partime Social Service Worker
Timberlyn Heights provides neuro-rehabilitative services to individuals with an acquired brain injury, traumatic brain injury, and other neurological/organic conditions.
The specialized services include: neurological, neuro-rehabilitative, and neuro-psychiatric services, as well as intensive behavioral management and treatment services necessary to promote recovery, improved quality of life, and, if possible, transfer to a less restrictive, community-based setting.
Social Service Worker work’s collaboratively with care team to coordinate care and provide education for residents related to any physical, emotional, or psycho-social needs while at the facility, discharge planning, and referrals to home services. Coordinate and communicate with VNA, TBI waiver programs, and other agencies. Also, works collaboratively with our psychologist to offer group therapy to our residents.
Must have experience in skilled nursing facility, LTC. Experience with DPH and CMS requirements.
Demonstrates an awareness of, a respect for, and attention to the diversity of the people with whom they interact (persons served, personnel, families/caregivers, and other stakeholders) that is reflected in attitudes, behavior and services.
Assists short term and long-term residents and their families in adjusting to their injuries, by dealing with life changes, grief, loss, placement, and depression through counseling and developing resident centered care plans with other members of the Interdisciplinary care team.
Assists with advanced directive planning in collaboration with nursing and medical services.
Responds to resident and family needs by advocating on behalf of the resident any wishes that are presented. Attends appropriate facility meetings on behalf of the residents such as Care plan meetings, Welcome meetings, Medicare meetings, etc.
Completes documentation per federal and state guidelines and sends required documentation to appropriate agencies, discharge locations, and care teams.
Educates residents and families regarding financial resources
Assists with orientation of new employees as it relates to resident rights.
