CHD Outpatient Behavioral Health Service Case Manager in Greenfield, Massachusetts
GREENFIELD Case Manager Description The Center for Human Development (CHD) is currently seeking an experienced and creative Case Manager at its Two Rivers Recovery Center in Greenfield. Two Rivers is a program for women in early recovery from substance use who are pregnant or postpartum mothers with their infants. The program supports up to 25 women and 5 infants in long term (6 to 12 months) residential treatment. The comprehensive clinical service is a trauma informed, harm-reduction model. The Case Managers is responsible for the development and implementation of person-centered treatment plans, facilitating evidenced-based therapeutic group, individual therapy, assistance with discharge planning, as well as community support coordination and collaboration. The Case Manager receives supervision from a professional, experienced Clinical Director. Qualifications: Bachelors Degree in a related field is required; experience in clinical positions; knowledge of substance use disorders and treatment; experience facilitating groups; one weekend day required.
