Assessment & Outreach Coordinator Job Type: Full-time - M-F - 8 hour shift Experience: Case management 2 years (Required) Language: Spanish (Preferred) Work Location: Hybrid remote in Holyoke, MA GENERAL SUMMARY As part of the interdisciplinary team, the Assessment/Outreach Coordinator provides high quality outreach and engagement to LTSS members. The Assessment/Outreach Coordinator provides education to enrollees regarding the benefits and purpose of the CP program, how the assigned CP will work with the enrollee, and the importance of engagement and participation of the member within the CP program. The Assessment/Outreach Coordinator is also responsible for conducting comprehensive health assessments and creation of person-centered treatment plans that reflect the whole health needs of the individuals served. In addition, the Assessment/Outreach Coordinator may provide care coordination assistance where needed. ESSENTIAL JOB FUNCTIONS* Provides telephonic and in-person outreach and education regarding the CP program which includes, but is not limited to: benefits and purpose of the CP program, how the CP will work with the member, and the importance of engagement and participate of the member within the CP program. Completes intake paperwork and any additional specific forms and documents that are required within required timelines. Conducts initial and annual assessments as required for members in ways that reflect respect and dignity for the individual and their health needs and services. These are conducted in-person in home or community locations as preferred by the individual serviced. Attains competence in motivational interviewing and utilizes this approach when working with ambivalence. As part of the care team may assist in monitoring the effectiveness of care plans and suggest changes as needed in order to promote positive health outcomes and responsible use of resources. Provide Care Coordination to members on an as-needed basis. Demonstrates willingness and ability to work with challenging and diverse enrollee populations. Demonstrates ability to collaborate with a team of interdisciplinary professionals. Demonstrates working knowledge of community resources and how to access them. Completes paperwork and any additional specific forms and documents that are required within required timelines. Completes documentation within 2 business days (reference Agency Documentation Policy). Has consistent and punctual attendance to required meetings as required by supervisor or designee Completes all other responsibilities as required by the Supervisor and/or Program Director. Completes other routine assessments as required by the program. PREPARATION, KNOWLEDGE, SKILLS & ABILITIES Broad knowledge of physical and mental illnesses, developmental disabilities, SUD, and the impact of social determinants on overall health preferred Satisfactory background check that includes CORI, SORI, SAMs GSA, Mass Health Exclusion List and OIG check. Valid driver?s license required Vehicle available during all working hours. Vehicle should be Registered and inspected in accordance w 540 CMR 4.00. Familiarity with Internet, email systems, electronic health records (EHRs), and databases.
