Description:

Position Purpose: We are seeking a passionate and organized Quality of Life Coordinator to join our team at Trinity Health Senior Communities at Mary's Meadow. This role involves planning, coordinating, and implementing opportunities for meaningful engagement for our elders based on individualized assessments.

What You Will Do:

  • Complete in-depth assessments of each elder to create personalized engagement plans that align with their life patterns and preferences.

  • Collaborate with elder assistants to ensure elders are engaged in both group and individual activities.

  • Resource individual rooms and homes with the necessary supplies for personalized engagement.

  • Organize and schedule a full range of individual and small group activities based on elder preferences.

  • Participate in interdisciplinary team meetings to enhance holistic and quality care of residents.

  • Facilitate Resident Council to comply with Resident Rights Regulations.

  • Follow all safety, security, infection control, and hazardous materials policies and procedures.

  • Maintain high standards of employee conduct in compliance with regulations.

  • Document resident engagement and activity attendance accurately.

  • Attend all required educational programs annually to comply with Department of Public Health Regulations.

Minimum Qualifications:

  • Completion of a Therapeutic Recreation Certification or equivalent experience.

  • Demonstrable knowledge of handcrafts, games, and exercise routines.

  • At least one year of experience working in a long-term care setting or training in directing group activities.

  • Proficiency in MS Word, internet, email, and electronic medical records.

  • Exhibits interest, enthusiasm, and patience in motivating elders.

Ministry/Facility Information: Mary’s Meadow at Providence Place features four separate houses, each accommodating 10 residents with private rooms and shared living spaces, connected to a central chapel. This setting provides a home-like atmosphere that empowers residents to be as independent and self-reliant as possible.
