Hilltown Community Health Center Community Health Worker (CHW) Supervisor in Huntington, Massachusetts

A day in the life of this role: The CHW Supervisor must have a strong and genuine commitment to the care of vulnerable people and communities. High quality supervision can lead to enhanced services for clients, increased staff satisfaction, decreased burn-out/staff turn-over, improved team relationships, and improved client/patient outcomes. Community Health Workers are public health workers who apply their unique understanding of the experience, language, and/or culture of the populations they serve in order to carry out one or more of the following roles: Providing culturally appropriate health education, information, and outreach in community-based settings, such as homes, schools, clinics, shelters, local businesses, and community centers; Bridging/culturally mediating between individuals, communities, and health and human services, including actively building individual and community capacity; Assuring that people access the services they need; Providing direct services, such as informal counseling, social support, care coordination, and health screenings; and Advocating for individual and community needs. The supervisor will oversee the recruitment and training of the CHW. He/she will also be responsible for daily supervision of the CHWs and annual performance evaluations. The supervisor helps with the assignment and management of the CHW?s caseloads, provides onthe-spot training and support, helps problem solve and helps obtain resources for the patients if the CHW is unable. If the CHW is providing home or agency-based visits, it is essential that the supervisor is easily available for back-up support and emergencies. In terms of professional development, quality supervision should facilitate movement to the next level of competence as well as facilitate self-confidence. Modeling these skills for staff can help CHWs to implement them with their patients/clients. What we need from you: ? Any combination of 3 or more years of health/social services experience and/or post H.S. education in a related field ? CHW Supervision Certificate strongly suggested, but will consider supporting candidates who display an aptitude for this work in obtaining certification qualifications ? Culturally and linguistically qualified to work within the population ? Demonstrated commitment to impacting at risk patients ? Ability to work independently and as a team member across multicultural settings ? Detail oriented with the ability to multi-task ? Computer literate, experience with electronic medical record preferred ? Strong time management, organizational and planning skills ? Reliable transportation required ?
