Cape Abilities Day Hab Developmental Specialist Lead in Hyannis, Massachusetts

Provides the Lead support & assistance to program participants in the areas of independent living, self-management within the community, social & independent living skills development & affective & behavioral management. Communicates with families & providers, as required. ESSENTIAL JOB FUNCTIONS Establishes & maintains a rapport with day habilitation program participants. Ensures & safeguards members? human dignity, rights & liberties. Plans & oversees participant services in accordance with day habilitation regulations in the areas of independent living, self-management within the community, social & independent living skills development & affective & behavioral management. Assists program participants in making & expressing choices based on assessments & other service need indicators. Creates goals & objectives. Plans program activities in accordance with those goals & supervises implementations of services created by interdisciplinary team (IDT). Liaison to program case managers to ensure documentation compliance & in keeping with the development of the Day Hab Service Plan (DHSP) goals. Review case management with team in assigned area to ensure implementation of goals. Attends, or assigns DHSM, team & ISP meetings for participants in the assigned area. Lead Participant group meetings to develop individual & group activity choices in activities & schedules. Meets on a regular basis with the individual participant to review personal goals & makes recommendations on changes through Day Habilitation case managers as necessary. Assists in the development & expansion of opportunities for community integration based on the needs, choice & wants of the program participants in keeping with Day Habilitation scope of service. Provides physical assistance to program participants, as needed, including, but not limited to, toileting, transferring & feeding. Adheres to all funding source regulations & Agency policies & procedures. Keeps management abreast of planned activities, as well as, any potential participant issues or needs, which are not being met. Establishes & maintains professional relationships with colleagues, management & community partners. Serves as mandated reporter for suspected cases of abuse &/or neglect to the Disabled Persons Protection Commission (DPPC). In addition, may perform one or more of the following functions: Fosters good working relationships with community partners & other organizations. Writes & develops participant progress notes & assessments, as well as, compiles daily, weekly, & monthly reports. Fosters maximum independence while decreasing support level in individual?s plan. Present as a positive role model for participants & model professional behavior for staff. OTHER DUTIES & RESPONSIBILITIES Attends all meetings, as requested. Performs additional duties, as assigned. PREPARATION, KNOWLEDGE, SKILLS & ABILITIES Bachelor?s degree preferred. Minimum 2 years prior experience in a community health setting. SUPERVISORY RESPONSIBILITY Maintains a weekly activity planner for the assigned program created by program staff & participants. Submits planner to the Day Habilitation Director for review & approval. Implements & assigns planned activities for program area staff to engage participants in a structured & scheduled, meaningful day in accordance with Day Habilitation scope of service. Assign case management & documentation to Day Habilitation Service Managers in program area & ensure accuracy & timely completion of that documentation. Lead area program staff planning meetings with focus on activity planning, participant goal development, strategies, & problem solving & goal achievement. PHYSICAL DEMANDS Frequent standing, walking, bending, & twisting. Ability to lift up to 50 pounds & to pull up/transfer people weighing 250lb
