State of Massachusetts Substance Use & Addictions Counselor - Adult Community Clinical Services (Human Services Coordinator II) in Hyannis, Massachusetts

The Department of Mental Health (DMH) is seeking an exceptional Substance Use and Addictions Counselor for the Adult Community Clinical Services (ACCS) program with the Cape Cod Site Office. The successful candidate will screen and assess person served on an ongoing basis regarding substance use/addictions issues and participate as a member of an integrated team to provide ongoing teaching and modeling to Teams/Group Living Environments (GLE) and provides direct interventions and supports to Persons Served with substance use/addictions issues. They will provide trauma-informed, person-centered community-based interventions to assigned caseload, participate in the development of Community Service Plans (CSPs) with assigned caseload and ensure appropriate linkage to substance use/addictions resources in the community. _Please Note:_ The work schedule will be first shift working8:30am - 5:00pm, Monday through Friday. The days off will be Saturday & Sunday. _Duties and Responsibilities (these duties and responsibilities are not all inclusive):_ * Functions as a Substance Use/Addictions Counselor for assigned persons on the Integrated Team and provides ongoing screening/assessments and reassessments for assigned individuals related to substance use/addictions issues, addiction risk and addiction treatment needs. * Provides competency-based, evidence-based, and best practice interventions related to substance use/addictions issues including: Screening, Brief Intervention and Referral for Treatment (SBIRT), Stages of Change model, Motivational Interviewing, and Harm Reduction. * Develops formulations that address the level of substance use and potential risk, the current impact of substances or potentially addictive behavior, and the Persons Served readiness for change. * Works collaboratively with ACCS LPHAs to provide input during the assessment period and assists with the drafting of the CSPs. * Provides community-based interventions to persons served and side-by-side consultation/direction to members of integrated team regarding substance use issues in order to ensure successful provision of CSP interventions, to coordinate hospital discharges, to facilitate family/group meetings, and to access community services and support through care transitions. * Provide and/or coordinate with Care Coordination Entities to ensure Persons have access to community-based clinical, self-help and peer, medication assisted treatment, and psycho-educational services that address addiction treatment and related issues (e.g., HIV, AIDS, Hepatitis C) and relapse prevention. * Coordinates discharge planning for dually diagnosed or substance active persons served by means of treatment planning conferences involving persons served, relatives, community caregivers and team members in order to insure the most appropriate living and aftercare arrangements. * Provides individual/group substance use counseling to Persons Served in ACCS. * Responsible for the coordination and completion of the Comprehensive Assessment within the 45-day timeframe that is outlined by regulation by meeting with the client, family and any other individuals that can provide information as well as by gathering and incorporating outside documentation. * Responsible for the development of an CSP for each person assigned that is completed within the 45-day time limit. The CSP development will include the person served, service providers and any other supports the person served would like to include. * Responsible to ensure timely documentation of person served progress and to ensure compliance with existing DMH regulations and program procedures. *Preferred Qualifications:* * Professional licensure in a human service field including but not limited to LCSW, LICSW, LMHC, License Alcohol and Drug Counselor (LADC) or Certified Alcohol and Drug Counselor (CADC). * Previous experience providing services to individuals with co-occurring disorders: substance use disorders and mental illness. * Previous experience working with people with chronic mental illness. * A current and valid Massachusetts Class D Motor Vehicle Operator’s license or equivalent from another state is needed for this function. * Given the population served, bilingual or multilingual fluency. __ _DMH Vision and Mission:_ The Department of Mental Health, as the State Mental Health Authority, assures and provides access to services and supports to meet the mental health needs of individuals of all ages, enabling them to live, work and participate in their communities. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience. Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.
