Therapeutic Mentor/Case Manager Hyannis Office The Fall River Deaconess Home is seeking a Therapeutic Mentor/Case Manager to join our Support & Stabilization team located in Hyannis, MA. The Therapeutic Mentor/Case Manager is responsible for providing direct services to youth & their families. The mentors support community-based service recipients as they participate in a wide variety of activities designed to prepare them for success in a stable living environment, including practicing pragmatic daily living routines, identifying & engaging in safe recreational activities, maintaining personal boundaries, & learning how to express themselves in a socially appropriate manner. Timely & accurate documentation of these activities is an important aspect of this individual?s role. Are you looking for a way to support youth & their families in the community? The successful candidate will: Work collaboratively with youth, families, funding agency & treatment team to identify & address the focal needs of the youth & their family Provide supportive services within youths? homes, school & community Coordinate access to basic needs (food, clothing, shelter, & utilities) while providing transportation if necessary Identify available community-based resources & manage referrals, engagement, follow-up & coordination services Attend & participate in supervision, staff meetings, trainings, conferences, & workshops to promote professional development Establish positive working relationships with youth & families Responsible for a caseload working with youth & families Provide strength-based services utilizing a \"wraparound\" model Provide crisis & interventions to youth & families in the home, community, & school Identify & develop natural supports with the youth & their family Participate in the creation & implementation of treatment plans Provide outreach & community-based mentoring which may focus on teaching personal skills related to self-care, social skills, education & pre-vocational attainment & engaging youth in age appropriate social & recreational activities Maintain record keeping requirements in accordance with agency standards & contractual obligations Maintain ongoing collaboration with other service providers & funding agencies Participate in appropriate clinical & administrative staff meetings & in-service trainings Travel within program catchment area (towns/cities in the southeast region) Schedule Sunday through Thursday OR Tuesday through Saturday Qualifications High school diploma (required) Bachelor\'s degree (preferred) Experience in an organization which provides services to families, adults & youth Valid Driver\'s License Proven oral & written communication skills Candidate must be mature, assertive, organized, & able to multi-task & prioritize Ability to understand, support, communicate with, & work with service recipients & their identified families Ability to work collegially with a variety of individuals inside & outside the organization Benefits We offer an exceptional benefits package that includes: Generous contributions to medical coverage, including deductible payments Comprehensive dental insurance Optional vision plans Paid vacation & sick time Life insurance Tuition assistance after one year of employment Employee support program Annual health insurance gym membership/weight loss reimbursement Discounted membership at Healthtrax Fitness & Wellness Center 403b retirement plan with employer base contribution plus matching Monthly staff appreciation recognition The Fall River Deaconess Home is an Equal Opportunity Employer (EOE).
