Amergis Healthcare Staffing is partnered with a much-respected facility in Cape Cod, MA who is currently seeking a Home Health Physical Therapist to assist them on a contractual basis.

Details of the position include:

Position: Home Health Physical Therapist

Location: Home visits to patients in the upper, mid, lower, or outer Cape Cod

Start Date: ASAP

Duration: 13 weeks (with likelihood of extension)

Schedule:

• 40 hours per week

• Monday – Friday

• Day shift (5x8’s)

• Plan to work 2 weekends during the 13-week assignment

• Occasional holidays

Experience/Contract Requirements:

• Prior home visit experience required

• Must be able to evaluate and start PT care

• Current Massachusetts PT license

• Current BLS certification

• EPIC experience preferred

• COVID vaccine not required (declination accepted)

Job Description:

Start of care and case management of a home-based caseload of a variety of patients (up to 6 units each day)
