Montachusett Home Care FT GSSC – Geriatric Support Service Coordinator in Leominster, Massachusetts

Position Overview: The Case Manager assists elders by providing assessments and comprehensive information so the individual and their family are empowered to make decisions for a life of dignity, safety and respect.

Responsibilities include: Conduct an initial assessment of physical, social, environmental and emotional status to determine needs and eligibility requirements within the established time frame guidelines. Completing reassessments as scheduled according to each of the Senior Care Options contract and level of member (i.e. level 1 and level 2). Authorizing appropriate services in accordance with the individual Senior Care Options policy and procedures; monitoring care plans to ensure needs are being addressed and services are being delivered as authorized. Advocating effectively on behalf of elders to assist them in availing themselves of the benefits and programs available via the Senior Care Options. Maintaining a thorough knowledge base regarding community resources and benefits provided by the Senior Care Options. Ensure all applicable information regarding elder and services is recorded timely and accurately in system.

A Bachelor?s degree in social work or a related field and two years? experience in casework, or related activity. Experience in a human service field is preferred. The Worker must be able to work with a diverse population, make mature judgements, possess good communication skills and maintain effective interpersonal relationships. Knowledge of basic medical, mental health, social service delivery systems, as well as acceptable record keeping practices is essential.

A valid driver's license and a legally insured and registered motor vehicle are required, a copy of current license and registration will be on file in the personnel record.

Compensation and Benefits:
