Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Licensed Behavioral Health Clinical Team Manager in North County Community Partners
Community Healthlink’s Behavioral Health Community Partner (BHCP) program is a new, innovative Care Management approach for MassHealth members with complex medical, behavioral health and social needs who are part of the new Accountable Care Organization (ACO) and or a Managed Care Organization (MCO).
Multidisciplinary BHCP teams will play a key role in providing high quality integrated care, functioning as a bridge between clients, behavioral health providers and medical providers. Team members will use their expertise to support individuals to navigate health and social systems to help members address social determinants health and improve overall quality of life and health.
The BHCP Licensed Behavior Health Supervisor will manage the BHCP Care Coordination Team. In this role, the BHCP supervisor will help with implementing this exciting, innovative service for MassHealth members with complex medical, behavioral health and social needs. The BHCP supervisor will have experience working in a care management setting with people living with SMI and or SUD. The BHCP Supervisor will assist the Director in developing procedures and workflows that are highly collaborative, instill hope, and most importantly, center on the Person’s strengths.
Responsible for managing an integrated team of Care Coordinator/Case Managers and RN, assessing Behavioral Health risks of team’s cases, and supervision of team performance.
Supervise the BHCP Team.
Provide clinical supervision to CP Team Care Coordinators.
Assist in monitoring BHCP metrics including quality, utilization, and engagement
Assist BHCP Director in implementation & coordination of CP activities, particularly in care management and in the development of Person-Centered Car Plans.
Develop & maintain relationships with primary care, BH, LTSS, & hospital providers
Demonstrate effective communication with consumers, staff, peers and other professionals.
Performs duties using recovery-oriented principles.
Demonstrate person centered strength-based communication with staff and consumers.
Knowledge of and ability to work effectively with community-based resources, referral networks, and public/private systems of care.
Encourages and supports diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, respect, tolerance, civility and acceptance toward all employees, patients and visitors.
Must have a Master's degree and an independent license in MA (LICSW, LMHC, Licensed Psychologist)
Must have a minimum of two years’ experience
Possess strong interpersonal skills and confidence
Able to exercise good judgment and is creative and proactive
Be flexible and able to multi-task
Current valid US-issued driver’s license and ability to provide a registered, inspected and insured automobile for work related purposes including travel between work sites
