Wellforce LPN in Lowell, Massachusetts
The LPN utilizes the nursing process in the planning, implementing and evaluation of nursing care to the patient. This individual collaborates with other members of the team in providing health information, maintenance and restoration. The LPN may collect data on the health status of the patient, in conjunction with other members of the health care team, for the identification of health goals.
Hours: 36 hours per week, Day Shift
Qualifications: Requires that the LPN has graduated from an approved practical nursing program (LPN). Requires current license as Licensed Practical Nurse. Basic CPR Certification immediately. CPR Instructor highly recommended.
• Conducts accurate data collection; identifies and prioritizes patient/family strengths, needs and priorities; implements, under the direction of an RN, the patient’s individualized plan of care; to meet mutually agreed-upon clinical outcomes. • Care is consistent with established policies, procedures and standards of care (follows algorithms, decision trees, and protocols in the delivery of patient care). • Identifies the normal course of illness in patient population recognizing the need to communicate with the RN to intervene when deviations occur • Accurately communicates patient information and thoroughly documents nursing actions and plan of care. • Seeks validation of clinical judgment from RN and other experienced clinicians. • Implements an effective transfer/discharge plan as directed by the RN. Caring Practices: Executes nursing activities that create a supportive, compassionate, and therapeutic environment for patients and staff, with the aim of promoting comfort and healing and preventing unnecessary suffering. Includes, but is not limited to, vigilance, engagement, and responsiveness of caregivers, including family and healthcare personnel. • Shows respect for patients/family dignity and privacy in all patient/family interactions. • Establishes and maintains mutual trust within the nurse-patient/family relationship.
