CircleHealth , a member of Tufts Medicine is seeking a Family Medicine or Internal Medicine Physician for a unique opportunity serving patients of Lowell General Hospital and Greater Lowell, Massachusetts . In this role, you will work toward preventing readmissions, coordinating comprehensive care coordination, provide same-day clinic availability, work in conjunction with mobile services and social determinants of health resources as needed.
The Transitions Clinic program is targeted toward patients with multiple chronic, complex conditions, high utilization of ED, inpatient and subacute facilities, and barriers to care

The Transitions Clinic aims to provide care to intensely medically complex patients in an outpatient setting to address any medical, social and care coordination needs that may be contributing to avoidable hospital utilization, with the aim to prevent 30-day readmissions, promote recovery and address barriers to care. Our goal is to build and cultivate a quality patient-provider relationship. We strive to listen to the needs of our patients, educating and providing a safe and confidential setting for any and all of your concerns.

How you’ll transform patient care:
Work directly with the Transitions Clinic RN to develop comprehensive care plans for patients.
Perform extensive chart reviews with attention to utilization counts and patterns, involved specialty providers, outstanding studies or follow-up needs, and gaps in care.
Provide initial comprehensive consultation with patient and/or family members or caregivers, either in person in clinic or via virtual Zoom visit with MIH paramedic in the home.
Ensure patient receiving evidence-based standards of care.
Connect with patients’ other providers to coordinate care and discuss discrepancies in plan amongst patients’ various primary and specialty, inpatient and outpatient providers.
Identify each patient’s individual drivers of utilization and address.
Identify and address any social determinants of health needs.
Be available for same day urgent visits either in clinic or via MIH.
Effectively collaborate with visiting nurse association and other community agencies and providers to provide oversight to patients’ entire spectrum of care.
Oversee Remote Patient Monitoring program.
Track performance measures for Transitions Clinic.

Who you are:
Board Certified in either Family Medicine or Internal Medicine
Current medical license in the Commonwealth of Massachusetts is preferred
Hospital Medicine experience preferred
Outpatient medicine experience preferred
A team player who is excited to work both independently and cooperatively with a wide range of stakeholders and disciplines to champion world class patient satisfaction and support.
